Total depth of the drill collars is 260. Drill pipe was set at 1680. Petroset pumped 150 sacks of ClassA cement down the drill pipe. Displaced with 9.6 barrels of fresh water. Cement top should be around 1300 deep. I found no violations at this time.

The drilling equipment is gone. The debris has been removed. The field in question is a bean field. Some leveling needs to be done and the electric from the well needs to be installed. The well is in the middle of a golf course. The greens have grass growing on them. The fair way is still bare. But the Golf Course will plant grass seed next spring. The drilling pits are closed. The ID sign is posted. This well does not pass preliminary restoration.

1331831172 2012-03-13 Production Wells No Violations On 3/13/2012, DNR Inspector Richard Skidmore completed a production well inspection status check for DNR Inspector Chris Grimm. The well is equipped with a pumping unit. The tank battery is location behind the barn. The lease road, tank battery, and well head locations are in compliance with 1501:9-9-05. Inspection passed.
1347290113 2012-09-08 Production Wells No Violations On 9/8/2012, Deputy Chief Robert Worstall notified me that he had received a call from Atlas Energy notifying him that one of Atlas Energys Energy tanks, a 201-barrel tank, had been struck by lightning during the night. I responded to the call at 11:00am on 9/8/2012. Upon my arrival, Brudley Excavating was on location digging a collection pit 150 northwest of the tank battery to collect the fluid that was released from the lightning strike. The area affected was 50 wide by 150 long in area. There was 53 (71.82) barrels ( bbls ) of fluid in the tank. 30-35 barrels of that fluid was contained in the containment dike. Atlas notified the Ohio EPA, Kurt Kollar. When Mr. Kollar arrived on location, we walked the storm water run-off tributary to Charley Run Creek. There was no show of oil at Charley Run Creek. Pander Trucking was on location to pull all fluids from within the containment dike area. They then moved to the pit in the pasture field and pulled all fluids from the pit. At that time, we decided to dig a long trench along the outside of the tank batterys northwest end, dig a sump hole and flush the tank battery with fresh water. After two flushes with Panders Water and Atlas Water, the dike was empty and the outside trench was empty. The complete northwest end of the pasture field was diked in case it rained overnight. There was also a siphon dam installed. They will wait until Monday, 9/10/2012 to come back and start the rest of the cleanup. Upon my arrival on 9/10/2012, they were in the cleanup process of removing and properly disposing of the contaminated soil. I will inspect the site daily.
1352129795 2012-11-05 Production Wells No Violations On 10/05/2012, I performed an inspection on the above location. On 9/8/2012, the 210 barrel tank was hit by lighting. The location has been restored and vegetation has been established, tank has been replaced with a 140 barrel tank. The tank battery consist of one 140 barrel tank, 35 barrel brine tank and a horizontal separator, the well is equipped with a pumping unit with a electric motor. I D signs are posted. No violations were noted at this time.
