I checked the area surrounding the horizontal target well location, which is 790 feet east of the well surface location. The house in question at 169 Shadyside Road is about 1360 feet from the horizontal target well location. The area does not have any depressions or signs of any surface damage from the fracturing of the well. It appears the well is not the cause of the sinkhole or driveway and sidewalk damage. |

A Mr. Tony Romeo from 151 Shadyside street in Boardman called and complained about the miss in his front yard. I called Mr. Romeo and left him a message that I would check the area out tomorrow. I inspected the south side of Shadyside street. Ohio Valley had dug there production line between the side walks and the street. New driveways of concrete were poured. But the dirt between the side walk and the street were no cleaned up. Slag from the broken up concrete was in the dirt. I called Ben Funderberg from Ohio Valley. He will have there landscapers there within the hour. I called Mr. Tony Romeo and let him know what was going on. He thanked me. I came back in around 1 hours time and found the landscapers working on the dirt areas. The landscapers stated it would take about 1 week to clean up and sow grass seed. Case continues. |

He was VERY mad about the drilling rig coming thru his street rather than the next street up, Shadyside, which was the original route for the rig. He stated that it was illegal to move in on the location because the pre-site says coming in on Shadyside, not Beechwood. He finally hung up on me. Mr. Riddick called Wayne Schalk, my supervisor. Mr. Riddick told Wayne the same story. Wayne called me to ask what was going on. I told him that I was sending in the revised pre-site inspection to the Beechwood Dr. instead of the Shadyside Dr. He told me to take the revised pre-site to the location. Dale Nething called and said that Mr. Riddick came onto the drilling site and stole the permit and the pre-site. I was going to bring up the revised pre-site. Mr. Riddick called Rick Simmers and told him the story. Rick called me and told me to shut the process down until all the paper work is in Columbuss hands. I called Ben Funderburg(landman at Ohio Valley) and told him to shut things down until a letter was written about the revised access road and a revised aerial map. On 11/15/07. Ohio Valley emailed Steve Optriza and Mike McCormac the revised access road and a letter about the change. The hard copy will be there on Friday, 16th November. The change was due to the high lines and the no parking on the north side of the street. I called Steve about the revision. He stated that Ohio Valley may continue to proceed on making the location. I called Ben and let him know about starting up again. Ohio Valley will start back up on Friday, November 16,2007. Case resolved. |

The trees that were in question were not blocking the ingress and egress of this location. The lease road is passable. I called Mrs. Sandra Jackson and explained this to her. I called Ted Hill Ohio Valley Field Supervisor and ask him to put some Do Not Block Entrance signs at the lease road. It appears that the residents use this for a parking area, no emergency vehicles could get to the tanks or well head incase of an emergency. |
