0265658150 2017-01-13 Production Wells No Violations I performed a status check on the above location. The well is in production. It is a constant flow system. It equipped with a GPU unit. ID signs are posted at the well head. All safety areas are marked and emergency shut downs are marked. No noted violations at this time. There is one central oil/water collection system with twelve 500 barrels tanks. There is also 1 compressors and various fluid knock out systems on this pad. At this point the gas enters into a midstream pipeline system. There was no pressure on the annuals at this time.

1379520201 2013-09-18 Drill / Deepen / Reopen No Violations On 9/15/2013, I was notified by Norman Wood, drilling consultant for Hilcorp Energy that they did not get cement returns to surface on the 9-5/8 intermediate casing job. The well was drilled to a depth of 2500. He stated that on 9/16/2013 that Baker Hughes would be on location to run a Differential Temperature Log survey to locate the cement top. Upon running the log they could only get to 1148 inside the 9-5/8 casing. Not knowing if there is a split or separated casing, Falcone Drilling tripped drill pipe back in the hole with a Tri-Cone drill bit and drilled out the cement. They drilled to a depth of 2443 that is 57 above the shoe. Northeast Region Supervisor Tom Hill and myself meet in the Uniontown Office and recalculated the complete cement job, all calculation were correct with Baker Hughes job report. On 9/17/2013 I was on location to witness the 9-5/8 casing integrity test. The hole was loaded with fresh water. STT Testing pressured the 9-5/8 casing to 2400 psi. and held this psi for 30 minutes without a pressure drop. This was recorded on a 60 minute rotation chart. After the pressure test was completed, Baker Hughes was on location to perform a Segmented Bond Log Survey. Baker Hughes logged the top of the cement to be at 1275 from KB (Drilling floor). From the shoe there is 1171 of cement. This satisfies the stated requirement of 500 above the shoe.
1380805649 2013-09-15 Drill / Deepen / Reopen No Violations I witnessed Baker Hughes cement the intermediate casing for Hilcorp Energy for ODNR Inspector John Fleming. Falcon Drilling drilled a 12.25 Hole #3 to 2,500 and set 9.625 casing at 2,500. Baker Hughes established circulation with fresh water. The operator pumped 50 bbls of gel followed by 872 sacks of cement. Baker Hughes dropped the plug and displaced the cement with 190 bbls of fresh water. I witnessed a trace of gel returned to surface and no cement. Circulation was never lost during the cementing operations. I immediately contacted ODNR Inspector John Fleming. John told Hilcorp to run a bond log to locate the approximate depth of the cement.
