1010884857 2016-02-26 Production Wells No Violations E-Mail request from Regional Supervisor Tom Hill requesting a compliance Inspection of this well. If well is in non-compliance to process the necessary paperwork and refer the well to the Orphan Well Program. Inspection on 02/26/2016 met with landowner she said well is inaccessible and she needs to show me where it is and advised me to call back in two three weeks when weather clears up to get access to well site.
1531330827 2018-05-16 Production Wells No Violations I conducted a follow-up compliance inspection on this well at the request of DOGRM Northern Regional Supervisor Tom Hill. My inspection found this well approximatley 400 feet west of Vandermark Road, along the northern property line of the property owned by Stone Fox Equestrian Center LLC.

The well is equipped with a stuffing box, 2-3/8-inch tubing and rods but no pumping unit. The well head valves were closed. The 2-3/8-inch diameter flow line travels east from the well head approximately 30 feet to the former 15 feet X 25 feet tank battery. The exposed end of the flow line is corroded and open. There are no production tanks within the earthen dike of the tank battery and this area is wet and overgrown with phragmites. The Division database shows that this well was drilled in 1981 to the Clinton Formation, deemed a dry hole and plugged back to the Berea. The plugging report on file shows that three (3) staged cement plugs were set off the drill rig from Clinton up to 500 feet.

1855253481 2018-05-18 Production Wells No Violations I conducted this follow-up compliance inspection on this well at the request of DOGRM Northern Regional Supervisor Tom Hill to complete Orphan well Referral. My inspection found this well approximatley 400 feet west of Vandermark Road on property owned by Stone Fox Equestrian Center LLC.

This well is equipped with a stuffing box with rods but no pumping unit. The well head valves are closed. The flow line runs east from the well head for approximately 30 feet and is open ended. This flow line ends in a 15 x 25 area that is overgrown with phragmites. This may have been the tank battery, however there is no storage equipment present.
