All rig-related equipment has been removed, and the location has been rough-graded. The gravel pad installed for parking remains on the location. Location is very wet and muddy.

This well is located in the wooded portion of the property. The areas disturbed during plugging operations have been graded and seeded and vegetation is established. The gravel access area was modified, per landowner request, and restoration of well site and access approved by landowner. Final Restoration completed and approved.

1341845901 2012-07-05 Administrative Inspection No Violations Received phone call from William Disbro, Fire Chief, Chatham Township in Medina County. He had received several complaints concerning a strong order of gas in this area over the last few days. Met with him on Thursday 07/05/2012 to investigate the problem. Well has 10 PVC pipe with 90 degree ell vented approximately 8 above surface level. Gas discharge is visible at top off pipe. This well has previously been assigned I&O CN 2010 08 035. I referred all info and case folder to Gene Chini and Norburt Lowder on 07/09/2012.
1403796549 2014-02-24 Orphan No Violations Follow-up inspection on 02-24-14 for contract compliance.
1. Contractor placed terra-mats over stream and around well site, Rig stage over well .
2. Contractor excavated around well casing and placed a plastic liner around casing.
3. Contractor establish total depth with swabbing tool which will be run through a lubricator (well
    head control) on 6 casing to a depth of 505 tripped out.
4. The contractor installed a BOP onto the 6 casing. Then ran 2.375 tubing a depth of 498.
5. Then Cementers attached to the 2.375 tubing then mixed pumped 35 sacks of Class A cement  
    with a weight of 15.4# to set bottom plug 498 to 300.
6. Then contractor pulled the tubing out and shut well in gas tight. 
7. Will be Logging tomorrow morning.
