0316873816 2012-01-27 Preliminary Restoration No Violations The water level had over topped the pit and had spilled out onto the ground. The field supervisor says the company had been emptying the pit by truck. I saw evidence of this in the fact there was a pump truck hose laying on the ground at the well site. The area got almost 2 inches of rain yesterday. I spoke with the field supervisor and told him he needed to keep the fluid levels down regardless of the weather. Pit shall be closed as soon as possible.

All pits used for drilling shall be closed two months after completion of a well. A 2 inch line was connected to a frac tank on site near the well. The line was temporarily being used to dispose of saltwater from the well down the annulus. Now that the permanent Annular Disposal setup is complete the temporary setup shall be removed.

An oblong area around the wellhead roughly 165 on each side and 36 wide in the front while 240 wide in the back is void of vegetative cover. Significant erosion and sedimentation is occurring along the lease road and effecting the foundation of the landowners barn. The barn is less than 20 downhill from the point of greatest erosion impact. The Photos taken. The owner or the owners agent shall grade or terrace and plant, seed, or sod the area disturbed that is not required in production of the well where necessary to bind the soil and prevent substantial erosion and sedimentation. Failure to comply with in the given time period will result in a material and substantial violation.
