2016-10-20 | Production Wells | No Violations | Status check follow up inspection on a incident that occurred on 10/17/2016 with oil being discharged from the wells tank battery flow line. The product migrated down and saturated a area under a recently constructed road bed leading to the Gulfport Claugus well pad. Crews from Ohio/ West Virginia excavating were on site addressing the clean up by digging out the affected road bed and road bed sub surface. All affected material is being hauled to a EPA approved landfill.
2017-10-23 | Production Wells | No Violations | On this day I was notified by Troy Harmon who indicated they had mobilized their service rig onto the well to begin the plugging process. I stopped by the well site and found the well is equipped with a 2.875 inner string and a 1.5 production string along with a production casing string not visible at surface. RBDMS records show the well as a Lime well with a drillers TD of 1,550, should be a shot hole completion. The 10 conductor indicated on the completion record is not present.
2016-10-17 | Production Wells | No Violations | On this day I was notified at 11:44 am by a company man with Gulfport Energy that the flow line from the tank battery was leaking. I responded to find that the 2 fluid load line from the tank battery had been severed during recent construction work by Ohio/West Virginia Excavating workers, and when the workers for Whitacre Enterprises opened the flow line to allow the oil to be picked up by the oil hauler, the oil was drained onto the earths surface and under a recently constructed road bed leading to the Gulfport Claugus shale pad. Gulfport HSE personnel were on site along with Division Personnel to manage the clean up. The product was contained to the area of the road bed. Inspection of the well found the well equipped with a 2.875 casing liner- 1.5 production tubing and pumping rods with the well set up to be produced by the means of a pumping unit with both production valves open. At the production fluid storage area 2-100 barrel capacity tanks are present in an established dike area.
2017-10-26 | Plug / Plug Back | No Violations | On site inspection to witness the well plugging process with Harmons Well Service rig on the hole and Formation Cementing. The well is equipped with 6.625 casing only and the company did recover 2.875 that was set as a inner isolation string on a anchor type set down packer. There was also 1.900 diameter tubing in the well which has been ran back in as a plugging string set at 1,539 The well bore total depth was marked at 1,546 and was producing out of the Keener Sand Formation. The well was also a shot hole completion. Formation Cementing set a 14.8 barrel absolute volume bottom plug consisting of 100 sacks of Class A cement with 1% gel and 1% calcium with a 10 barrel gel spacer ran ahead along with 10 barrels of fresh water and gel ran behind the plug. The well circulated at the tail end of the bottom cement plug being pumped and the plug was displaced with 3.8 barrels of Gel and water. The tubing was then pulled up to 1,000 and a 300 isolatin plug set above the 1st plug. The tubing was then pulled up to 300 and the last plug tagged with the rigs wirelie before settting a surface plug from 300 to surface.
2018-06-18 | Final Restoration | No Violations | Final Restoration finds the area of the well location has been disturbed by pipeline construction and will be reclaimed once again by the pipeline crew. The well plugging contractor did the initial restoration. All equipment has been from the well site. The production tank is shared with other producing wells on the lease and will remain in service to those wells.
