Upon inspection of the well head the following equipment was observed present at the well head- 7 surface casing- 2 production tubing with the production valve open and the well set up to produce openly through the tubing. The tubing valve was open at the well head however no sales meter was present in the area associated with the said well. No production has been reported for the last 2 consecutive reporting years. No well identification is present and the 7 casing is rusted off at ground level. I spoke to the producer by phone and he has indicated he is scheduled to flow test the well and turn it back in line in the next 30 days. |
1484217404 | 2017-01-11 | Plug / Plug Back | No Violations | On site inspection to witness the well plugging process with a service rig on location owned by Chuck Dye and Formation Cementing. At surface there is a 2 tubing string set on a formation packer and 6.625 production casing. A 1 tubing string was ran in to 1,725 and a 11 sack plug set to isolate the formation below the packer, the 1 tubing was then pulled from the well and JET Wireline then ran in the hole and tagged the top of the 1st plug at 1,640 and the 2 was then shot off the formation packer at 1,625 with 1 joint of 2pulled to 1,609 and a 200 plug set over the top of the Berea. The 2tubing was then pulled up to 1,289 and a 400 60 sack plug was set to cover the Keener with the Keener correlated at approximately 1,200 to 1,211. A 200 plug was set to cover the bottom of the 6.625 set at 535. A surface plug was set from 372 to surface with cement staying up to surface. Also noted was gel spacers were ran between each plug and the well circulated during each plug set through the 2 tubing with the cement contractors indicating lift pressure during the setting of each plug. |

2141044726 | 2017-01-11 | Plug / Plug Back | No Violations | I was on location with Inspector Bill Stolarick to witness this well be plugged by Formation Cementing. We set 4 plugs in the well one at the bottom of the hole for 70 feet in 1 inch diameter tubing to isolate the packer in the bottom of the hole, the 1 inch diameter tubing was then removed and JET Wireline Services shot the 2 inch diameter tubing off at 1625 and one joint of 2 inch diameter tubing was pulled and a 200 foot plug was set through the 2 inch diameter tubing to finish off the bottom hole plug, one across the Keener and ground elevation for 400 feet, one at the bottom of surface for 200 feet, and one at the surface of the well for 350 feet. |
