0169122439 | 2017-03-16 | Administrative Inspection | No Violations | Administrative inspection report to document the phone conversation with Ed Crum with CGO Inc who was the last know producer. Mr.Crum indicated that he had reached and agreement through the legal system with the landowner Kathleen Hasselbarger and the well would be registered in the future with the intention to plug the well. |
0808084368 | 2015-03-02 | Production Wells | Violations Noted | Status check inspection as part of complaint # 4951 filed by landowner Jane Percel on 02/27/2015. Upon inspection of the wellhead the following equipment was found-6.250 production casing- 2 tubing and pumping rods with the production valves open and the well set up to be produced by the means of a pumping unit. At the production fluid storage area 2-100 barrel production tanks are located in an established dike area. Inspector will note that the well contract pumper Troy Harmon reported the well can only be produced during the warmer months due to the production fluid water from the shallow formation freezing in the winter. It is also noted that the well is owned by CGO Inc owner # 2849 and is not registered with the Division. Instructions are as follows -Register the said well. ODNR Monroe Co Inspector Bill Stolarik 740-507-6592. |
1476366338 | 2016-10-13 | Administrative Inspection | No Violations | On 10/12/2016 the landowner identified as Kathleen Hasselbarger contacted the Divisions Uniontown office and filed complaint # 5447. I returned her call at 15:30 pm with the following being documented. Mrs. Haselbarger indicated a local producer identified as Ed Crum was operating a well on her property without a lease and she had taken legal action to have him removed from the property. Mrs.Haselbarger now wants the well plugged and would like to register the well and file for a plugging permit. I provided her with the Division Surety and Bonding contact information for Patty Nicklaus. |
1508313444 | 2017-10-18 | Plug / Plug Back | No Violations | On site inspection to witness the well plugging process with Harmons Well Service rig on the hole and Formation Cementing. Upon inspection of the well it was observed to have 5.187 diameter casing present only at surface. The well completion records only reflect as to Geographical Formation (Injun) and not the well completion type or how much, such as Nitroglycerine and amount of quarts. The service rig has confirmed total depth at 1,695 and has ran 2 diameter 8 round thread tubing to a depth of 1,691. Formation Cementing then pumped 6 barrels of fresh water followed by 10 barrels of gel spacer then 3 barrels of water before starting the 19 barrel absolute volume cement mix consisting of 85 sacks of 1% gel cement. The well circulated during the initial stages of the bottom cement plug. The tubing was pulled up to 700 and the cement tagged with the service rigs wireline at 1,202 . At that point the tubing was ran back in to 1,187 and a 300 plug set to isolate the inside of the 5.187 . The plug was 8 barrels absolute volume and consisted of 35 sacks of class A cement with 1% gel and 1% calcium additives. The tubing was then pulled out of the hole and per my request due to the 5.187 casing annular space not being tight at surface the 5.187 was ripped at 300 and the well bore cemented from 300 to surface both inside and outside the 5.187. No other casing strings were present. The 5.187 casing was attempted to be parted at 300 by the means of a casing ripper with telegraph to surface but no separation. The 2.375 was then ran back into 300 and the well bore topped off with cement. Also resolved on this day was complaint # 4951 filed by landowner Jane Percel on 02/27/2015 and violation # -808084368. |
1527612821 | 2016-04-05 | Production Wells | No Violations | Compliance notice # -808084368 follow up inspection found the well registration has not been addressed by the producer CGO Inc who has identified themselves as the owner of the well. There has been an injunction filed against CGO Inc not allowing them to enter the property. The well is idle and I have expressed concern to Harmons well service that the production tanks fluid levels are not being monitored and the tanks were not emptied. |
1529424618 | 2018-06-19 | Final Restoration | No Violations | Status check inspection found the well site restoration meets ORC 1509.072 with vegetation established and erosion control in place. All equipment has been removed. |
