Production fluid waste water was observed flowing from the knock out tank and into the ravine. Samples were taken by Inspectors Bill Stolarik and Andrew Corder and turned into the Cambridge lab. Ohio EPA , Division Supervisor Rocky King and Division Hydrologist Kyle Tharp were notified of action taken and findings. Also on 8-28-2012 notification verbally by telephone was made to PP&Gs main office and corrective action was agreed upon by both producer and myself, Lead inspector Bill Stolarik . Inspector will note that the permit number for the 2nd well on lease is as follows 3411165735 and inspector is unable to enter report into RBDMS database at this time for that said well due to the fact that it is not in the system yet. |
