1423765911 2015-02-12 Production Wells Violations Noted Producing well status check inspection found the following present at the well head 6.250 production casing along with 3 production tubing and rods with the production valves open and the well set up to be produced by the means of a pumping unit. A single 100 barrel production tank is present with a fluid knock out separator on top. It was observed on this day that there is a pit below the 100 barrel tank that the fluid knock out separator is being allowed to drain into with a visible oil sheen present, chloride levels were tested using low and high Quantab strip tests with the low the levels exceeding the low level strip tests and reading 1323 parts per million on the high level strip tests. There is also signs of long-term pollution and contamination at the well head where the stuffing box has leaked over a period of time. On the day of my inspection I met on site with Dean Christman with the following instructions given as follows - Immediately empty and remove the rusted out leaking 100 barrel production tank and remove the contaminated soil, recover the standing fluid located in the dike. Erect a containment dike capable of holding 110% of the largest vessel in the dike. The fluid knock separator is allowable but can not be constructed in such a way as to allow product to drain in any way other than into a gathering tank. The producer has 30 days maximum to complete the above tasks. ODNR Inspector Bill Stolarik 740-507-6592.
1425996952 2015-03-10 Production Wells No Violations Follow up inspection to the violation # 1423765911 issued on 02-13-2015. Upon inspection of the production tank it was observed that the tank was no longer leaking fluid. Contact was made with the well owner Dean Christman @ 10:14 am on this day and he indicated that the well was shut in and not pumping to avoid any further fluid loss and a new tank has been purchased for the well. We agreed on this day to allow time for the weather to cooperate before constructing a new tank pad.
1426673581 2015-03-17 Production Wells No Violations On this day it was observed upon inspection that the production tank that was leaking has been removed and the area remediated. A new tank pad has been built with a new 100 barrel tank placed in use. Compliance Action # 1423765911 issued on 02/13/2015 has been considered resolved on this day.
1515595334 2018-01-10 Production Wells No Violations Status check inspection found the well equipped with production casing- tubing and pumping rods. There is both tubing and casing production flow lines attached to the well head with valves present in line and in the open position. The production facility consists of a single 100 barrel steel production tank along with a 1,600 gallon plastic brine tank. The producer has erected an earthen dike around the production facility. Also noted is the producer has reported commercial production but no brine disposal.
