1323092343 | 2011-11-28 | Production Wells | No Violations | On day of inspection well was complete with pump jack. Tank battery contents included: one 110 bbl oil storage tank and a mechanical separator. No violations noted. |
1342183600 | 2012-07-13 | Production Wells | No Violations | Inspection of the well head found the following equipment, 4.5 production casing, 2.375 tubing & rods with the production valves open and the well being produced by the means of a pumping unit. A 100 barrel tank and separator along with a gas meter with a current chart are present in a dike impoundment that will need further evaluation to meet SPCC rules. |
1531143614 | 2018-07-09 | Plug / Plug Back | No Violations | Status check inspection on the well plugging process found a service rig on the hole owned by Unlimited Oil Field service. The well had cast iron bridge plugs previously set in the 4.5 production casing. The plugging contractor has ran a 2.375 diameter upset workover string into the well bore with a drill bit on the bottom of the string. A mud pump and power swivel is on location along with a 1/2 round service tank to circulate into and out of. I have made 2 unsuccessful attempts to visit the well site while crews are present and working. |
1532020077 | 2018-07-19 | Plug / Plug Back | No Violations | Status check inspection found a service rig on the hole owned by Unlimited Energy Service out of West Virginia. The rig is drilling out the cast iron bridge plugs and frack sand with a mud pump and power swivel, the down hole equipment is 2.375 diameter upset tubing and a 3.750 diameter tri cone tooth bit. They initially reported to of sat down on sand fill up inside the 4.5 diameter casing at 1,500 and have was sand out of the hole down to 1,852 and now believe they are drilling on a cast iron bridge plug. They are circulating with fresh water and keeping the 1/2 round steel above ground tank to a proper level with fresh water from a nearby stream by the means of drawing it out with a vacuum truck and transferring it into the 1/2 round. Multiple stages were complete in the well bore and the well will be cleaned out to the original total depth before plugging shall commence. |
1536757174 | 2018-09-12 | Plug / Plug Back | No Violations | On site inspection to witness the final stages of the well plugging process only. Previously a company identified as Unlimited Energy Services out of West Virginal lost a drill bit -stem - and set of jars in the hole while attempting to drill out a (2) cast iron bridge plugs set in the hole at 1,800 and 2,300. The 1st plug was drilled out at 1,500 and the company became stuck while drilling out the 2nd plug at 2,300. After weeks of fishing unsuccessfully the company submitted a proposal to plug the well from 2,300 to surface. Division Supervisor Mike Moreland and myself reviewed the plan and approved it. The lower section plugged was not witnessed due to me being assigned to another project in Harrison County. The company set the lower plugs and had Appalachian Wireline Service free point the 4.5 diameter production casing and shoot it off at 450. The bottom of the 8.625 diameter surface casing was set at 306, On this day I arrived at 7:00 am and witnessed Formation Cementing establish circulation and pump cement through the 4.5 diameter production casing cut off at 450. The well was plugged solid from 450 to surface with an absolute volume of 29 barrels of Class A cement weighing 14.7 pound per gallon and a 1.36 yield. The 4.5 diameter casing was recovered from 450 to surface and the well topped off to 30 below grade with cement. |
1563174280 | 2018-06-20 | Plug / Plug Back | No Violations | On this day, I am conducting an inspection per request of ODNR Inspector William Stolarik. Unlimited Energy Services is in the process of rigging-up on the well, and spotting their equipment, in preparation for the plugging procedure. (2) water trucks (1) pump truck, and (1) half-round are present on location. I spoke with Operation Supervisor, Mike Sprouse, and they have been notified that there are pre-existing bridge plugs in the hole that will need to be drilled out, prior to plugging the well. They anticipate to start pulling rods and tubing on Friday (6/22/18), shut down for the weekend, and commence with drilling the plugs on Monday (6/25/18). It was indicated that Inspector Stolarik and myself will be notified with daily reports from Mr. Sprouse. |
