0560428252 2016-03-17 Production Wells Violations Noted Status check inspection in response to complaint # 5276 received on 03/14/2016 by the landowner Ron Dapkuliak. On the day of my inspection the following was observed at the well head- 4.5 production casing -2.375 production tubing along with a production tree on the tubing and pumping rods in the tubing with all the production valves found to be in the closed position and the well set up to be produced by the means of a pumping unit. A single 100 barrel production tank was present on location near the well head with no owner identification or earthen dike present, no gas sales meter was present on location or near the well and the last reported commercial production by the registered producer was 2011. The property owner indicated the production tank was full and was concerned that the tank may leak in its current condition. Instructions are as follows -The well owner/operator shall submit a written plugging schedule to (Bill Stolarik) at (5880 Memory Road Zanesville, 43701) within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
0808622785 2016-09-21 Administrative Inspection No Violations On 09/21/2016 I accompanied Inspector Bill Stolarik to G&Rs office to discuss with Dick Sulzberger the lack of response to the unresolved violation issued against the said well. Inspector Stolarik informed Mr. Sulzberger that they needed to submit a application to plug the well and schedule the well to be plugged.
1197681689 2017-08-08 Plug / Plug Back No Violations On site with inspection Bill Stolarik to witness the plugging operation on this well. The bottom hole perforations were covered up on this day. The cement calculations were verified and the job went as planned. The cement will be tagged tomorrow and Appalachian Wireline will be on site to shoot the casing that will be recovered from the well bore.
1437332970 2017-07-11 Production Wells No Violations Follow up administrative inspection to violation # -560428252 and complaint # 5276. G&R Contracting has a service rig on a second well on the property and has began the plugging process on 3411122299-0000. Upon completion of the plugging process on that well they will move over and plug the said well. The company per my request did empty the 100 barrel production tank last year. I have met with G&R Contracting and reviewed the said plugging process.
1466009780 2016-06-15 Administrative Inspection No Violations Follow up administrative inspection to violation # -560428252 and complaint # 5276. On this day I stopped by G-R Contractings Lewisville office and spoke with Dick Sulzberger in regards to the unresolved violation. Mr. Sulzberger indicated they planned on plugging the well. I informed Him they needed to apply for a plugging permit and address the violation.
1483611179 2017-01-05 Administrative Inspection No Violations Follow up administrative inspection to violation # -560428252 and complaint # 5276. A plugging permit was issued on 11/01/2016 and the well is scheduled to be plugged this year.
1502209048 2017-08-08 Plug / Plug Back No Violations Follow up to violation # -560428252 and complaint # 5276. On this day I was on site to witness the well plugging process along with ODNR Inspector Andrew Corder. Formation Cementing and G&R Contractings service was the contractors on location. The well was equipped with 4.5 production casing 8.625 surface casing and 2 diameter 10 round thread production tubing which will be used for the plugging string. The 2 diameter tubing was ran into the well bore and sat down on fill up inside the 4.5 casing at 3,280. On this day we set a single column of cement in 3 stages between the depth of 3,278 and 2,000 to cover the perforated zones indicated on the Companys completion records as the Gordon -30ft -Berea and Weir. The 1st 2 plugs were 7.1 barrels of cement consisting of 32 sacks of class A cement with 1% gel and designed to be 426 in length. The 3rd plug was calculated with 50% cement excess and consisted to be 10 barrels of class A cement. All cement was 1.26 yield and 15.2 pound per gallon cement weight verified by weight of scale. The cement will be tagged on this day with the service rig wireline tools.
1502297456 2017-08-09 Plug / Plug Back No Violations Follow up to violation # -560428252 and complaint # 5276. On this day I was on site to witness the well plugging process along with ODNR Inspectors Andrew Corder and Tom Thomas. On this day we witnessed a service rig on hole owned by G&R Contracting along with Appalachian Wireline Service and Formation Cementing. Appalachian wireline ran in and tagged the top of the previous plug set at 1,696 and cut the 4.5 off at 1,130. A 16.8 barrel plug was then set to isolate the bottom of the 4.5 and the top of the 4.5., the plug consisted of 75 sacks of cement. The 4.5 diameter casing was pulled up to 172 and a 8.5 barrel surface plug set with cement to surface and staying up to surface. All violations and complaints considered resolved on this day.
1910272795 2016-09-24 Administrative Inspection No Violations On 09/21/2016 I stopped by G&Rs office accompanied by Division Staff member Jimmy Wiles. I discussed with Dick Sulzberger the lack of response to my violations and the need to apply for plug permits for the said wells and multiple other wells that have violations issued against them. It has been indicated that G&R Contracting has had the lease assignment forfeited through the legal process for the said well due to the lack of production. The unresolved violation is # -560428252 and complaint # 5276.
