0129045112 2017-07-18 Work Over Reconditioning No Violations On site with Inspector Bill Stolarik to witness Timco Well Service utilizing a power swivel with 2.875 inch diameter to drill out the big mouth frac plug that was set in this well bore at a depth of 2350 feet. G&R Contracting representatives were also on site to aid in the drill out process.
0427589816 2018-02-26 Final Restoration No Violations Status check inspection found the well site meets ORC 1509.072. All violations and complaints are considered resolved.
0751559768 2017-07-11 Production Wells No Violations Follow up administrative inspection to violation # 1121111381 and complaint # 5276. G&R Contracting has a service rig on the well and has ran tubing into the well bore and sat down on a cast iron bridge plug below the Berea formation. G&R has been directed to drll out the plug and plug the well from the bottom up.
1121111381 2016-03-17 Production Wells Violations Noted Status check inspection in response to complaint # 5276 received on 03/14/2016 by the landowner Ron Dapkuliak. On the day of my inspection the following was observed at the well head- 4.5 production casing -2.375 production tubing with all the production valves found to be in the closed position and the well set up to be produced by the means of a plunger lift system. A single 100 barrel production tank was present, no gas sales meter was present. With the last reported production by the registered owner being for the year 2009. Instructions are as follows -The well owner/operator shall submit a written plugging schedule to (Bill Stolarik) at (5880 Memory Road Zanesville, 43701) within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

1398317642 2017-07-26 Plug / Plug Back No Violations On site inspection to witness the well plugging process with G&R Contractings service rig and Formation Cementing. A power swivel unit was previously brought in and the previously set completion plugs drilled out to a total depth of 3,626. The objective for the 1st day is to isolate all perforated and completed zones from the top Berea perforation at 2,391 to the bottom perforation exposed in the Ohio Shale at 3,844. The rig has ran 1.900 diameter tubing into the well bore to a measured depth of 3,200 and Formation Cementing shall set a total of 3 plugs 436 in height between the depths of 3,600 and 2,291. Before setting of the plugs the well bore was circulated bottoms up with 9.0 pound per gallon brine to achieve a static level. Then 1st 2 plugs were set consisted of 31 sacks of class A cement with 1% gel. On the top plug we ran 20% excess on and 37 sacks total. 14 tubing joints were pulled between the plugs before recovering the remainder of the tubing from the well bore on this day. The next days work shall be to the extent of bringing a wireline logging truck and tagging the top of cement then finding free point and cutting the 4.5 casing off and plugging the well on out to surface.
1430528536 2017-07-18 Plug / Plug Back No Violations On site inspection to witness the drill out of completion frack plug set previously in the well bore at 2,350. G&R Contracting has their service rig on the hole along with Timcos power swivel and mud pump unit. The company rigged up and established well control. I verified the Blow Out Preventer was in place and installed correctly to verify surface well control. Multiple issues with the power swivel unit delayed the job from beginning.
1500647574 2017-07-21 Plug / Plug Back No Violations Status check inspection on the pre-plug preparation work to the effect of drilling out the completion baffles set in the well bore. The company rig is on the hole being assisted by a power swivel and mud pump unit contractor identified as Timco Incorporated. They are on their 3rd day of working on the well and have drilled down to approximately 3,247 and have sit down on another object in the well bore. The well bore is building around 800 pounds pressure over night but is becoming static after loading the hole and circulating on it with 9 pound per gallon brine with the brine weight verified by weight of scale. The company rigged up the power swivel and well head control and will continue working their way to bottom. Upon reaching the bottom of the well bore they will rig down the swivel and pull the 2.875 diameter workover string then proceed to run 1.5 diameter tubing into the well bore and plug the well.
1501166093 2017-07-27 Plug / Plug Back No Violations 07:00 -On location to witness the well plugging process with Appalachian Wireline-Formation Cementing and G&R Contractings Service rig. On this day Appalachian Wireline ran in and tagged the top of cement on the previous plug set at a depth of 2,358, the last perforation was at 2,391 which put us with only 33 coverage over the top perforation. The well was then bond logged and 1.900 tubing ran back in and a 200 plug was set on top of the last plug to properly isolate the top of the last perforation, the plug was from 2,336 to 2,136. The plug was 2.12 barrels of class A cement with a 1.26 yield and displaced with 5.6 barrels of fresh water. The well is circulating out the 8.625 surface casing also and the bond log indicated a hole in the 4.5 below the surface casing at a depth of 1,238 and material behind the casing from 1,122 to 1,142, free pipe was marked at 1,100. An additional 200 plug was set between the depths of 1,300 and 1,100 to cover the hole in the 4.5 at 1,238. The 4.5 was shot off at 1,110 and a 60 sack plug set through the 4.5 casing to cover the rip and bottom of 8.625 surface casing. The 4.5 was then pulled up to 160 and a surface plug set. All product was removed from the plugging permit immediately upon the completion of the well plugging. The previous compliance notice identified as 1121111381 is considered resolved on this day.
1510555739 2016-06-15 Administrative Inspection No Violations Follow up administrative inspection to violation # 1121111381 and complaint # 5276. On this day I stopped by G-R Contractings Lewisville office and spoke with Dick Sulzberger in regards to the unresolved violation. Mr. Sulzberger indicated they planned on plugging the well. I informed Him they needed to apply for a plugging permit and address the violation.
1691444754 2016-09-21 Administrative Inspection No Violations On 09/21/2016 I accompanied Inspector Bill Stolarik to G&Rs office to discuss with Dick Sulzberger the lack of response to the unresolved violation issued against the said well. Inspector Stolarik informed Mr. Sulzberger that they needed to submit a application to plug the well and schedule the well to be plugged.
1830126144 2016-09-24 Administrative Inspection No Violations On 09/21/2016 I stopped by G&Rs office accompanied by Division Staff member Jimmy Wiles. I discussed with Dick Sulzberger the lack of response to my violations and the need to apply for plug permits for the said wells and multiple other wells that have violations issued against them. It has been indicated that G&R Contracting has had the lease assignment forfeited through the legal process for the said well due to the lack of production. The well has a unresolved violation identified as # 1121111381 and complaint # 5276.
2082301243 2017-06-28 Plug / Plug Back No Violations G-R Contracting plans to plug the well at the beginning of next week. The well is equipped with 8.625 inch diameter surface casing set at 997 feet, 4.5 inch diameter production casing set at 3910 feet, and 2.375 inch diameter tubing at an unknown depth. The wells pay zone was the Berea and the Ohio Shale and was completed through perforations set from 2391 feet to 3844 feet. G-R Contracting was on location with their service rig. During my inspection today I witnessed the 2.375 inch diameter tubing being tripped out of hole. Plugging calculations and procedures were discussed with G-R Contracting. The bottom hole plug will be set on Monday 7/3/17.
