A single 100 barrel production tank was present, no gas sales meter was present. With the last reported production by the registered owner being for the year 2009. Instructions are as follows -The well owner/operator shall submit a written plugging schedule to (Bill Stolarik) at (5880 Memory Road Zanesville, 43701) within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. |

A power swivel unit was previously brought in and the previously set completion plugs drilled out to a total depth of 3,626. The objective for the 1st day is to isolate all perforated and completed zones from the top Berea perforation at 2,391 to the bottom perforation exposed in the Ohio Shale at 3,844. The rig has ran 1.900 diameter tubing into the well bore to a measured depth of 3,200 and Formation Cementing shall set a total of 3 plugs 436 in height between the depths of 3,600 and 2,291. Before setting of the plugs the well bore was circulated bottoms up with 9.0 pound per gallon brine to achieve a static level. Then 1st 2 plugs were set consisted of 31 sacks of class A cement with 1% gel. On the top plug we ran 20% excess on and 37 sacks total. 14 tubing joints were pulled between the plugs before recovering the remainder of the tubing from the well bore on this day. The next days work shall be to the extent of bringing a wireline logging truck and tagging the top of cement then finding free point and cutting the 4.5 casing off and plugging the well on out to surface. |

The well bore is building around 800 pounds pressure over night but is becoming static after loading the hole and circulating on it with 9 pound per gallon brine with the brine weight verified by weight of scale. The company rigged up the power swivel and well head control and will continue working their way to bottom. Upon reaching the bottom of the well bore they will rig down the swivel and pull the 2.875 diameter workover string then proceed to run 1.5 diameter tubing into the well bore and plug the well. |

On this day Appalachian Wireline ran in and tagged the top of cement on the previous plug set at a depth of 2,358, the last perforation was at 2,391 which put us with only 33 coverage over the top perforation. The well was then bond logged and 1.900 tubing ran back in and a 200 plug was set on top of the last plug to properly isolate the top of the last perforation, the plug was from 2,336 to 2,136. The plug was 2.12 barrels of class A cement with a 1.26 yield and displaced with 5.6 barrels of fresh water. The well is circulating out the 8.625 surface casing also and the bond log indicated a hole in the 4.5 below the surface casing at a depth of 1,238 and material behind the casing from 1,122 to 1,142, free pipe was marked at 1,100. An additional 200 plug was set between the depths of 1,300 and 1,100 to cover the hole in the 4.5 at 1,238. The 4.5 was shot off at 1,110 and a 60 sack plug set through the 4.5 casing to cover the rip and bottom of 8.625 surface casing. The 4.5 was then pulled up to 160 and a surface plug set. All product was removed from the plugging permit immediately upon the completion of the well plugging. The previous compliance notice identified as 1121111381 is considered resolved on this day. |
