1426436314 2017-11-17 Plug / Plug Back No Violations On site inspection to witness the well plugging process with G&R Contractings service rig on the hole along with Formation Cementing. Upon inspection the well was observed to be equipped with 4.5 diameter production casing and 8.625 surface casing. The rig has pulled the 2.375 diameter tubing and pumping rods and ran the tubing back in hole open ended to a set down depth of 3,561. The bottom 200 plug inside the open hole was set in the following order 35 barrels of brine was pumped in attempt to load the hole followed by 15 barrels of 6% gel spacer to fill the 361 area between the bottom plug and the plug set to isolate the bottom of the 4.5 followed by a 8.4 barrel class A plug weighing 15.6 pound per gallon with 40 sacks of cement and 2% calcium additives, 1.5 barrels of gel was ran during the 10 barrel gel displacement. The 2.375 tubing was pulled up to 3000 and a plug set to cover the bottom of the 4.5 and the Devonian Shale area perforated between the depths of 2,757 and 2,633. The company man indicated he wanted ample coverage over the top shot so 2,300 was our calculated target point of the top of cement. A 9.8 barrel absolute volume plug was pumped consisting of 65 sacks of cement with 2% calcium and cotton seed hulls additives with the well circulating during the initial stages of the pumping of the cement. The tubing was then pulled out of the hole and Appalachian Wireline brought in to Bond Log the well and shoot off the 4.5 casing. I requested they tag the top of cement and discovered the 2nd plug did not stay in place and was tagged at 2,934 the free point of pipe was marked at 1,750 but not shot off. The company man on site indicated that they were going to run tubing back in and re set the 2nd plug. Approval to witness the plugging process past 40 hours was denied. The plug was not witnessed by ODNR staff. Also Historically the well was attempted to be converted to a salt water injection well with previous Acid treatment and stimulation test impleme
1499745435 2017-11-20 Plug / Plug Back No Violations Status check inspection on the well plugging process accompanied by ODNR Inspector Tom Thomas. On this day I witnessed G&R Contractings rig on the hole along with Appalachian Wireline Service. The service rig tagged the top of cement inside the 4.5 diameter casing at 2,325 with the top 4.5 casing completion perforation at 2,633. Appalachian Wireline previously logged the 4.5 diameter casing on 11/17/2017 ant the top of cement on the annular side of the 4.5 diameter casing was marked at 1,750 and shot off at 1,572.
1510929090 2017-11-17 Plug / Plug Back No Violations On this day, I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a detailed report. GR Contracting has a service rig on the hole and Formation Cementing has a bulk truck and pump truck on location. 14 barrels of gel were pumped ahead of the bottom plug that consisted of 40 sacks of cement, followed by 1 barrel of gel. Then, tubing was pulled up to 3000 feet and pumped 65 sacks of cement. Appalachian Wireline arrived on location to run a bond log.

Appalachian Well Surveys is on location attempting to cut the 4.5 inch diameter casing. Casing was shot off at 1610 feet. The casing will be removed from the hole and additional plugs will be set.
1511273729 2017-11-21 Plug / Plug Back No Violations On site inspection to witness the well plugging process with G&R Contractings rig on the hole and Formation Cementing. The lower plugs to isolate the open hole below the 4.5 and the perforated section have been set ant the cement top tagged at 2,310. The 4.5 diameter casing was shot off at 1,610 and one 38 joint of casing was pulled up to 1,572 and a 250 cement plug was set calculated off of a 7.875 diameter borehole. The plug was an absolute volume of 15 barrels of cement consisting of 67 sacks of Class A cement with 1% gel, the plug was displaced with 21 barrels of salt water and the 4.5 casing pulled up to 530 and a 13.46 barrel absolute volume plug set across the bottom of the 8.625 surface casing, the plug consisted of 0 sacks of class A cement. The 4.5 casing was then pulled up to 200 and a final surface plug set.

The bottom of surface plug was set from 530 feet to 330 feet with 61 sacks of cement. A 200 foot top plug was set and topped off with 60 sacks of cement.
