0250915035 2015-09-16 Production Wells No Violations Status check inspection found the following present at the well head -4.5 production casing -1.5 production tubing with the production valves open and the well set up to be produced by a plunger lift system. At the production fluid storage area a single 100 barrel production tank is present along with a mechanical separator and gas sales meter with no current sales chart present in the meter and the 2 flow line from the separator and tank is rusted off and laying on the ground inside the dike.

1553690797 2019-03-27 Plug / Plug Back No Violations 03/27/2019 On site inspection to witness the well plugging process with a rig on the hole owned by Stampede Well Service out of Marietta Ohio and Formation Cementing. The rig has pulled the production pumping rods and 1.900 diameter tubing out of the hole and ran the 1.900 diameter tubing back in the hole open ended to 1,951, the 4.5 diameter production casing total depth is 1,969 and the well was completed in the Squaw Sand with completion perforations between the depths of 1,880 and 1,860. The hole was loaded with production brine water the previous day and the well bore circulated by Formation Cementing prior to the pumping of the 6.3 barrel volume bottom hole cement plug that consisted of 30 sacks of class A cement weighing 15.6 pound per gallon and with a yield of 1.18, the plug was displaced with 3.9 barrels of production brine water. The 1.900 tubing was pulled out of the hole and the 4.5 diameter casing filled with brine water and the well bond logged by Appalachian Wire Line Service. The top of the bottom plug set was tagged at 1,560. The 7 diameter surface casing was set at 390 and the 4.5 showed tight on the log up to 520. The 4.5 was shot off with a electric casing cutter at 470 and was free at that point. The well bore was circulated by Formation Cementing with brine water before starting to pump the 7 barrel bottom of surface plug that consisted of 45 sacks of class A cement. Heavy cuttings circulated out of the hole ahead of the cement. The 4.5 diameter casing was pulled up to 200 and a final surface plug set with cement to surface and staying up to 30 below grade.
