0040838743 2018-01-02 Production Wells No Violations On this day, I found the well equipped with 4.5 inch diameter casing with the valve open, 1.5 inch diameter tubing with the valve open, and produced by means of a pumping unit with tubing, rods, and a gas engine. There is (1) 100 barrel steel tank, drip tank, vertical separator, and domestic meter all contained within an earthen dike. The tank is properly identified and the production records are current.
0577817767 2019-05-09 Final Restoration No Violations On this day, I am accompanying ODNR Inspector William Stolarik in reference to the final restoration. Please reference his comments for a detailed report. Troy Harmon and Derick Harmon with Harmons Well Services are onsite, along with landowner Ken Kraynak.
0690821214 2018-11-20 Plug / Plug Back No Violations I am onsite to follow-up with Mr. Troy Harmon, of Harmons Well Services, in regard to the surface plug being set. The surface plug was unwitnessed. Mr. Harmon indicated that prior to setting the plug, cement was tagged at a depth of 209 feet, and the 4.5 inch diameter casing was shot-off at a depth of 185, and pulled from the well. Formation Cementing set a 200 foot surface plug inside the 7 inch diameter surface casing, consisting of 40 sacks of Class A Cement with a 1.18 yield and a weight of 15.6 pounds per gallon. The cement in the wellbore appears to be stable at the surface.
0759938107 2018-11-19 Plug / Plug Back No Violations On this day, I am accompanied by ODNR Inspector William Stolarik. I am onsite to witness the plugging of the well with Harmons Well Services and Formation Cementing. Harmons has a service rig on the hole with 1.5 inch diameter tubing set at a depth of 2,332 feet. The bottom plug (Gordon perforations) was set from a depth of 2,332 feet to 1,932 feet with 30 sacks of Class A cement with 1% calcium gel, and a weight of 15.2 pounds per gallon with a 1.26 yield. It was displaced with 4.8 barrels of fresh water. It was indicated that the Berea was perforated, so 12 joints of tubing were pulled to 1,924 feet. The second plug (Berea perforations) was set from a depth of 1,924 feet to 1,524 feet. The plug consisted of 30 sacks of Class A cement with 1% calcium gel, and a weight of 15.2 pounds per gallon with a 1.26 yield. It was displaced with 3.8 barrels of fresh water. Tubing was then removed from the hole, and Jet Wireline ran a Bond_CCL and indicated that the 4.5 inch diameter surface casing could be shot-off at the first free point, at a depth of 645 feet. The rig was unable to pull the casing, so another attempt was shot at a depth of 581 feet. The casing was able to move, so the rip/ bottom-of-surface plug was set with 40 sacks of Class A cement with 1% calcium gel, and a weight of 15.2 pounds per gallon with a 1.26 yield. The well had circulation, and at 3 barrels into a 6 barrel displacement, circulation was lost and the hole locked up and held pressure at the pump truck. Harmons attempted to pull the casing, and was unsuccessful. I spoke with ODNR Southeast Supervisor Mike Moreland, to explain the situation, and it was advised to have Harmons tag the top of cement, locate one collar above cement, shoot-off and remove the existing 4.5 inch casing, and set the surface plug.

1542645703 2018-11-19 Plug / Plug Back No Violations On site inspection accompanying Inspector Tom Thomas for training purposes. The well was plugged from total depth of 2,388. I was on site to witness the 2 bottom plugs to cover the producing formation.
1544453994 2018-12-10 Final Restoration No Violations On this day, I am conducting an inspection to assess the progress of the final restoration process. The storage tank and production equipment appears to be removed, but pieces of plastic flowlines are present. The affected area has been graded, mulched, and seeded. Diversion drainage ditches have been placed within the slope of the lease road. Pictures were taken. A follow-up inspection will be conducted.
1556549604 2019-04-29 Final Restoration No Violations On this day, I am conducting a final restoration follow-up inspection. A call was received by ODNR Inspector William Stolarik from landowner Ken Kraynak stating that he was unsatisfied with the grading of the site in regard to the restoration. My inspection found pits filled, debris removed, equipment removed, approximately 50 percent vegetation established, and no substantial erosion. A follow-up inspection will be conducted to verify additional vegetation being established. Whitacre Enterprises will be notified. Multiple pictures were taken.
1557412152 2019-05-09 Final Restoration No Violations On this day I met on site accompanied by ODNR Inspector Tom Thomas along with the landowner Ken Kerynak and contractors Troy Harmon and Derek Harmon with Harmons Well Service. The purpose of the meeting was to discuss the landowner Ken Kraynaks complaint regarding the restoration. Mr. Kraynak has indicated he is displeased with the final grading of the well site and tank battery area. Mr. Kerynak has also indicated he would like stone from the entry point of the lease road onto the property to the 1st existing gate. It was explained to Mr. Kraynak that the producer has meet the requirement of the restoration plan submitted to the Division and the well site along with the lease road currently meets ORC 1509.072 b with vegetation established and erosion control in place. However the initial restoration plan did indicate a pre existing entry point was being used. It was agreed that Harmons Well Service would return and grade the well site and tank battery area and re seed the area. In addition stone would also be placed upon the entry point of the lease road. Mr. Kraynak was also advised that any additional disturbance past this point was not enforceable by the Division. Photographs were taken on this day.
