0085819071 | 2012-10-03 | Plug / Plug Back | No Violations | Inspector on site to witness next stage of plug job, Pipe was cut using a wire line cutter @ 4264. And company could not pull casing but had good circulation so a 200 plug was set from 4264 to 4064 and pipe is slated to once again be cut on 10-04-12, Company will tag plug and cut at the required depth that the next plug is to be set in hopes of the rig being able to pull the lighter amount of casing . |
0308325522 | 2012-09-17 | Plug / Plug Back | No Violations | Inspector stopped by well site and checked on the company preparing to plug the well, The tubing and pumping rods have been removed from the well bore and a borehole deviation survey is being run on the well bore by Blue Dot Services. Plugging is scheduled to commence on 09-20-12. |
0801702946 | 2012-09-20 | Plug / Plug Back | No Violations | Appalachian wire line ran in to tag cement top on bottom plug. The depth was at 5356.They shot 4.511.60lb /ft. casing off at 4264. The well would not blow down. The pressure was so strong on backside that the rig could not break mandrel off and pull 1 joint of casing. The company is plugging well due to horizontal permit being issued. Formation was pumping 8.9lb/gal of brine on well but gas would blow fluid back to surface. I suggested to pump heavier brine such as 10lb/gal. The company did and the well was still blowing. The company called after I left and asked if they could produce well down and still get the drilling permit for the horizontal. Per conversation with Steve Opritza this could be performed. They will have to apply for plug back permit with the formations from the Warren to the Weir until reaching of the bottom of the 7 surface at 1818. Then after the well produces down the company will plug the well to surface. |
1005013132 | 2010-05-19 | Production Wells | No Violations | I stopped at the Virco office and talked to Tom Palmer and Bob Hanshumacher and Ben Smelzter.I explained that the pit liner has to go and can not be used no more. The plastic liner is starting to deteriorate. They stated that they will put the fluid in tanks. Remove the old liner and dig the pit larger so it will accommodate the new well proposed for the top of the hill. Then they will reline the pit and make liquid tight. I stated that the deputy chief was down the day before. |
1114014858 | 2012-10-04 | Plug / Plug Back | No Violations | Inspector on site to witness well plugging process with the following activity taking place, The previous plug was tagged @ 3973 and the pipe was cut @ 3165 with one 44 joint of 4.5 casing then pulled from the well bore and the Warren zone isolation plug set. Upon commencing to pull the casing the rig experienced a power tong equipment failure and the casing was pulled manually until it was clear of the plug and the hole was circulated clean above the plug. With at least a 4 hour delay ahead and the inspector out of hours for the week contact was made with my supervisor Rocky King and the decision was made for the inspector to request the company to finish plugging the said well on affidavit, proper zonal isolation plug placement had previously been reviewed and documented by Inspector Pat Shreve, company representative Ben Smeltzer and myself Inspector Bill Stolarik. |
1233754218 | 2012-09-19 | Plug / Plug Back | No Violations | I accompanied Bill Stolarik and Pat Shreve during this inspection of the bottom hole plug. Formation ran 35 sacks of cement. No violations were noted. |
1348175054 | 2012-09-19 | Plug / Plug Back | No Violations | Company is placing bottom plug at 5923-5450.Thru 2.375tubing that is 8 round thread. The Company Formation cementing is setting the plug of 35 sacks. I was overseeing job along with Bill Stolarik and Andrew Corder. Good circulation. |
1352908053 | 2012-11-14 | Final Restoration | No Violations | The well has been plugged and casing cut off and the plugging pits filled in compliance with 1509.072, The production tanks will need to be removed if not intended to be used for another well in the area. |

1901017119 | 2010-05-19 | Production Wells | No Violations | Tom Tugend and I stopped by the site to look at the frac pit that was left and intended for an adjacent well that was going to be a horizontal well. The pit was falling in and stubs from small trees were piercing the pit liner. Tom and I agreed that the pit needs to be removed. |
2066402969 | 2009-12-28 | Preliminary Restoration | No Violations | Well is supplied with 7cas,4.5cas and 2 3/8 upset tubing. There is a 210bbl tank and a 100bbl tank. Dike and id are inplace.I called Tom with Virco on the Marcellus pit. Ben will call back next week on when they will drill the horizontal well and use this pit for the frac. |
