The site meets ORC 1509.072 with vegetation established and no substantial erosion. The well is equipped with 4.5 inch diameter production casing and 1.9 inch diameter tubing with the production valves open. It is produced by means of a pumping unit with rods and a gas engine. The production facility consists of (1) 100 barrel steel tank, drip tank, vertical separator, and domestic meter all contained within an earthen dike. The tank is properly identified and the production records are current. |

Harmons Well Services is on location scraping and removing any contaminated soil within the affected area. I spoke with Troy Harmon during the inspection. He indicated that from what has been recovered, he estimates the spill to be considerably less than the original 20 barrels reported. The plan moving forward is to mulch, seed, and establish new vegetation in the affected area. Additionally, there is a syphon dam constructed at a gathering point, approximately 1.25 miles downstream, prior to a culvert, and hard booms are inserted after the culvert, as a secondary recovery point. Rain events have recently occurred since the incident, and minimal sheen is witnessed at the syphon dam. |

At 3:45pm I received a call from ODNR Dispatch explaining that approximately 20 barrels of production fluid had entered into a stream. OEPA and Division of Wildlife had been notified. Harmons Well Service is onsite with diapers, booms, and a vac truck. The fluid was released from a transfer pump near the wellhead and traveled approximately 100 feet downhill, before entering an unnamed stream (39.73846, -81.15958). Then it traveled approximately 1.25 miles downstream, where Harmons Well Service positioned booms and a vac truck at a culvert in the stream (39.72137, -81.15987). It does not appear that the fluid had traveled into the confluence of Brister Fork/Town Fork Creek. Diapers and booms were used in additional locations upstream, as well. I contacted Koy Whitacre to discuss the incident and the steps being taken to address the issue. A follow-up inspection will be conducted. |
