0345864517 2018-11-13 Production Wells No Violations On site inspection found 3 wells on the well pad with all 3 being Marcellus shale wells. The wells are equipped with artificial lift technologies and producing off the production tubing. The 5.5 diameter production casing pressure is 100 pounds per square inch (psi). There is a riser pipe on the 9.625 well head section with 100 psi present. Proper well identification is posted and there is an additional well pad between the 3 wells and the production facility. The production fluid storage facility has been reduced to (3) 500 barrel tanks situated inside a polymer lined steel containment dike area.

There is activity around the well pad supporting a second pad adjoining the well pad the said well is on. The company has submitted a simultaneous operations plan that indicated they will put a cage over the wells and establish a perimeter around the wells with barricades in place. Both are being followed out. The well produced into a midstream operation on a nearby pad.

I stopped by area of the complaint.Opposum creek at Rt.7 west about 100yards.Two trucks with hoses in the water. The trucks were loading water and getting ready for cement job. There was no evidence of sheen at all. I tried to call the complaintant but not successful. Marsha is with Wayne National forest.

1217841735 2014-03-14 Production Wells No Violations Compliance notice # 1971162819 & # 1300727520 follow up inspection found that the areas mentioned with erosion and sedimentation problems in the initial violations have been addressed along with the well head flange issue, the water impoundment has been emptied and rebuilt with a new liner installed and is standing full of water, additional wells have now been drilled on the well pad causing other areas of the site to have the soil disturbed noting a new production facility being added on the West side of the property to accommodate the Ormet #1 & #2 shale well pads, the preliminary restoration will not be approved on this day but the initial violations have been addressed.

1382078735 2016-02-18 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the initial well pad construction does have vegetation and erosion control established. There is a fresh water impoundment in place adjacent to the drilling pad. A separate production facility is located near the well pad, this facility will service multiple well pads in the direct area owned by the current registered owner (Triad). Documenting the well head - the well head is complete with the tubing production valve in the open position. There is a hydraulic emergency shut down device located in the production tree outlet. The company has annular pressure gauges on all annulis sections of the well head and the company has a authorized representative periodically checking and documenting all annuli pressures. Addressing the production facility- A total of (8) 400 barrel production tanks are located inside a polymer lined metal containment dike area with (2) low pressure vapor recover units also present inside the dike area. A (6) station commercial tank load out facility is located on the production pad along with a naturel gas compressor and gas processing equipment.
1410282977 2014-09-09 Preliminary Restoration No Violations On the day of my inspection accompanied by ODNR staff and Production Supervisor Rodney Boron, I witnessed that the well was equipped with Cameron casing and tubing wellheads along with a Christmas tree valve assembly and a sand trap controlled manually to a 300bbl frac tank in containment. Production equipment included a GPS Unit, a compressor unit, dehydrator unit, and a fuel skid. Tank battery consisted of 8-500bbl tanks and 2 let down towers all within containment. Also on site were 4 combustors and a pig launcher. The site passed preliminary restoration according to ORC 1509.072A.

1453394270 2016-01-21 Production Wells No Violations On this day I was on site as a follow up inspection to a report of a incident on the production well pad. I received the initial report on 01/20/2016 at 18:15 pm from a Rodney Boron who is a field Supervisor for the company. Upon inspection it was observed that a Heat exchanger tube in the B-Tex system possibly failed causing a over pressurization of the secondary recovery system that has a inlet pressure of 800 psi and outlet pressure of 12 ounces. In the course of the event a small fire occurred on the B-Tex building from the discharged fluid and was extinguished by Triads personnel on site with a 8 lb chemical fire extinguisher. The B-Tex system is a secondary recovery system that works between the gas dehydration system and the glycol heaters on the well production gas processing system. The system manufacturer (Exterran) is scheduled to arrive tomorrow 01/22/2016 to evaluate and re-certify the system. The 3 producing wells along with all the production equipment is shut down at this time. The Monroe County EMA did arrive on site while my inspection was taking place.
1498733922 2017-08-22 Production Wells No Violations Producing well status check inspection found the well equipped with a casing and tubing well head and a 2 production sales line set up to sell product off the tubing well head. The sales line has a hydraulic Emergency Shut Down with no electrically controlled actuation solenoid. The well head is leaking gas from the B-section between the 9.625 & 13.375. On site was Triad Production Foreman Derik Higgins who was informed to install flow lines and pressure relief valves on all well annulis. Mr. Higgins was informed that the gas was not to dispersed into the atmosphere and a permanent recovery system must be installed. The well head assembly gauge showed 1000 psi at surface.
1611328587 2017-08-22 Production Wells No Violations Arrived on location with Inspector William Stolarik. Please refer to his comments for a complete report. Upon inspection the wellhead was leaking from the 9 5/8 inch diameter casing with 1000 psi on the gauge. It was recommend to recover the leaking gas with a low pressure recovery system and divert it to the flowline.
