I received the initial report on 01/20/2016 at 18:15 pm from a Rodney Boron who is a field Supervisor for the company. Upon inspection it was observed that a Heat exchanger tube in the B-Tex system possibly failed causing a over pressurization of the secondary recovery system that has a inlet pressure of 800 psi and outlet pressure of 12 ounces. In the course of the event a small fire occurred on the B-Tex building from the discharged fluid and was extinguished by Triads personnel on site with a 8 lb chemical fire extinguisher. The B-Tex system is a secondary recovery system that works between the gas dehydration system and the glycol heaters on the well production gas processing system. The system manufacturer (Exterran) is scheduled to arrive tomorrow 01/22/2016 to evaluate and re-certify the system. The 3 producing wells along with all the production equipment is shut down at this time. The Monroe County EMA did arrive on site while my inspection was taking place. |

On site was Triad Production Foreman Derik Higgins who was informed to install flow lines and pressure relief valves on all well annulis. Mr. Higgins was informed that the gas was not to dispersed into the atmosphere and a permanent recovery system must be installed. The well head assembly gauge showed 1000 psi at surface. |
