0930512051 | 2013-10-08 | Preliminary Restoration | No Violations | Status check inspection found the well is in production with proper identification posted on the well head and location entry point, There is 2- 400 barrel production tanks present in a metal containment, along with 2-30,000 gallon propane storage tanks to store the processed material on site, there is also a compressor on site. Status check on the preliminary inspection found the prior requested site remedial work complete and vegetation established along with erosion control in place, the site is enclosed in fence along with a fresh water impoundment enclosed in a fenced in area. The restoration is still under Division supervision from the prior violations. |

-Marty Thalken,Rick Piles,OEPA-Steven Cassidy,Tom Harcarik,Corp of Engineers-Matt Mason,Hired Engineer from Penn E&R Mike Kearns,Monroe Soil and Water Dave Shot.Andrew Corder assisted with the inspection. The main goal was to establish the head waters of the affected stream west of the drill pad. The company has hired Lang to bring equipment in to fix the roads from the State Route 536 back to the pad entrance. Lang is also reconstructing the creek on the west side by extracting large rock from south of the water impoundment and placing the rock over the drains coming off of the hill side. The benches are then built using shale rock and compacting them in. The benches are built at a 2to1 slope with 1% decline back in the hillside. The large hill to the east of the pad is being pulled down and will place that material over the shale rock benches. The large slip material was placed on the hillside to the north west and will be placed below the water impoundment. The affected creek area is approximately 186in length and about 4 foot wide. The company plans to have the site done with in 6 weeks. The Corp of Engineers and OEPA are going to talk on the purposed penalties and the after the fact permit.OEPA stated that the company can still keep moving forward and the Engineering firm will make new plans and specs noting the changes being done to the site. | |

1971440111 | 2012-07-10 | Production Wells | No Violations | On the day of my inspection the well was shut in. Langs excavating were working on the slip area and the high wall. They had progressed approximately 3/4 of the way up the slip area with two benches made for stability. The high wall was completely gone and the remaining dirt will be constructed back to a 3-1 grade. |
