0809512090 2015-06-18 Production Wells No Violations Status check inspection found the well is in production with the production valves in the open position and 3 sets of 2-210 barrel production tanks on location in a plastic lined steel containment area supporting the said well along with the 5 other producing wells on location, there is also a total of 6 gas processing units on location supporting each well on the pad. Due to the recent rainfall standing water was found present on location and tested for chlorides with Low Level Quantab strip field strip tests finding no Chlorides on the entire location over 121 parts per million on this day. Also no sheen was observed as present.
0887524019 2015-01-28 Production Wells No Violations Administrative inspection to document the well drilling history. The said well was originally spudded as 3411124302-0000 and during the initial drilling process the well bore total drilling depth was reached but due to hole conditions the well bore collapsed before the drill string was pulled completely from the well bore and part of the drill string was left in the hole. The well bore bottom section was abandoned and plugged back to the vertical section and re permitted as 34111243020100 in which another well bore was lost once again and plugged back a 2nd time. A additional permit was issued as 34111243020200 in which the well bore was once again lost and plugged back into the vertical section for a 3rd time. A 4th permit was issued as 34111243020300 and the well was eventually completed as this API # and exists as producing today. I spoke with Steve Opritza in the Divisions Permitting section and he confirmed the well history as recorded to be accurate.
1315490804 2011-09-06 Drill / Deepen / Reopen No Violations On day of inspection I was accompanied by inspector Brett Farnsworth. Decker Rig 7 was on site and had completed the surface job on 09/05/2011. The rig was connecting their annular Blowout Preventer (BOP). The BOP was tested as well as the choke line. All pressure tests were successful. This rig is utilizing a closed loop system.
1348324190 2013-12-11 Production Wells No Violations The well drilling and stimulation process has been completed and the company is hooking up the completion testing equipment with the well being in production within 24 hours.
1360677764 2013-02-12 Production Wells No Violations Upon inspection of the tank battery site it was observed that the steel lined dike was standing full of what appeared to be rain water. Inspector made note of this to the company man on site and asked that the water be removed. Notification was also made to Josh Hinton, HGs operations manager.

1600192887 2013-02-13 Production Wells No Violations Inspection of the well head found the well in production and selling gas off of the production tubing. 2-400 barrel production tanks are present in a man made dike area. Inspector will note that my request on 02-11-2013 to have the rain water removed from the dike impoundment has been meet and the dike is now empty.
