0294244979 2019-01-28 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (7) producing wells, gas processing units, (6) 400-barrel steel production tanks within steel containment, (4) 30,000 gallon horizontal condensate pressure vessels in containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing does not have a gauge, and the 13.375 inch diameter surface casing has a pressure of 0 psi.
0489598037 2013-10-21 Drill / Deepen / Reopen No Violations Notification was received from the company man on site over the past weekend that they had reached their total drilling depth at 11260 and are running and cementing the 5.5 production casing, upon inspection on this day the well bore drilling is complete and the rig is skidding to 3411124381-0000.

Once the plugs are drilled out the Utica section shall be plugged back in order for the well to be completed in the Marcellus formation. The company man on site Rick Pace called later this day and indicated they will be ready to plug tomorrow at 12:00 pm.

1369143025 2013-05-18 Plug / Plug Back No Violations On site inspection to witness the setting of the 2 bottom hole plugs over the Utica and Marcellus shale sections, Upon arrival the well bore production casing had 3500 psi on it and a 180 barrel 10.8# fluid pill was used to kill the well, Schlumberger well services are the cement contractors scheduled on location for 9:30 AM but did not arrive until 3:30 Pm, A 900 bottom hole plug was set and a 449 Marcellus plug was set from 6612 to 6163, with the intention of setting a cast iron bridge plug on top of the cement and ripping the pipe at the annular cement top and setting a 600 rip plug properly covering the Marcellus on the Annular of the 5.5 that did not have proper coverage on the initial well bore completion process.
1369167747 2013-05-21 Plug / Plug Back No Violations On this day we ran tubing and a tooth bit in to check the top plug set on 05/18/2013, The plug was to be set from 6612 to 6163. The plug was not in place or encountered. The company circulated the hole clean and shall place a cast iron bridge plug below the bottom perforation and will re cement this section accordingly.
1369316877 2013-05-22 Plug / Plug Back No Violations On site inspection to witness the plugging of the Marcellus formation, A cement plug was set inside of the 5.5 production covering a perforated section that was tested to check the Frack gradient, The plug was set from 6773 to 6163 and a cast iron bridge plug was set at 6160 at the approximate height of the cement on the backside of the 5.5 annular space. The casing will be shot off tomorrow just above the cast iron plug and a 400 cement rip plug shall be set to complete the Marcellus zonal isolation.
1369702612 2013-05-23 Plug / Plug Back No Violations On site inspection to witness the company attempting to cut the 5.5 production casing at the top of the cement above the Marcellus production zone. The service company was unable to pull stretch on the casing to get the casing out of the well head slips due to the casing being set in the slips 120,000 pounds over string weight by the drilling rig. The company continued to work on the project the next day with no success.
1369833593 2013-05-28 Plug / Plug Back No Violations Status check inspection on the plugging back of the Utica Formation. I will note that the Halliburton cement bond log that the company had on file showed a cement top on the backside of the 5.5 production casing at 6170 and this is the depth we have been working off of trying to cut the casing and isolate the top of the Marcellus. On this day Grey wire line company re-bond logged the well and found the cement top to be at approximately 3850. The company will run in the hole with tubing and finish setting the top of the Marcellus plug inside the 5.5 producing casing on 05/29/13 and then finish plugging the well accordingly.
1369865532 2013-05-29 Plug / Plug Back No Violations On site status check inspection to witness the setting of the plug inside of the 5.5 production casing on top of the Marcellus formation. Upon arrival I found that the rig crew had set the tubing string down in the last cement set in the casing plugging it up as not to allow circulation. The rig crew removed the plugged tubing joint and will run back in and set a 450 plug.
1370016173 2013-05-31 Plug / Plug Back No Violations Plugging inspection found the company trying to cut and pull the 5.5 production casing. The casing was cut with a jet cut wire line cutting tool at 3600 and a collar buster at 3422 with good telegraph to surface on both cuts but the casing will still not pull due to it being set in the slips by the drilling rig at 135,000# over string weight. A set of hydraulic casing jacks were brought in to try and lift the casing out of the slips. A mechanical plug was set at 3750.
1370141807 2013-06-01 Administrative Inspection No Violations I received phone notification from the company man on site Rick Pyles that they were able to free the 5.5 production casing from the well head slips with the assist from the casing jacks by lifting 400,000 pounds on the casing and pressuring 1100 psi on the annular on the casing. The company will try tomorrow to cut the casing again at 3422 with a collar buster explosive shot.
1370306729 2013-06-03 Plug / Plug Back No Violations On site inspection to witness the kick & rip plug being set on this day. The casing was ripped at 3422 and pulled out of the hole. 2.375 tubing was ran back in to a depth of 3433 and a cement plug set from 3433 to 2376. The well was circulated at 2376 and the plug shall be tagged tomorrow morning.

1642064757 2015-01-14 Preliminary Restoration No Violations Shale well preliminary restoration status check inspection found the said well has had the drilling and completion process completed and multiple wells are producing on the well pad with the following production related equipment in place on the location. There are 7 total wells on location each with individual gas processing units and a total of 4 -3,000 gallon 150 psi cylindrical vessels used for condensate along with 4-400 barrel production water tanks. Preliminary Restoration passed.

1929694370 2013-05-15 Plug / Plug Back No Violations Status check inspection found Mercer Well Svc running tubing into the hole to drill out composite bridge plugs set during a Frac Gradient test. The current tubing depth is 6,000 with the 1st plug set @ 6,250. Once the plugs are removed the company will pull the tubing and remove the drill bit and run in open ended and set the bottom plug over the Utica formation and then plug the well back accordingly.

Pressure test of the 9.625 previous casing string was done on 10/14/13 to a pressure of 2800 psi with no recorded loss, A formation leak off and integrity test was also done and recorded.
