1178132032 | 2018-10-30 | Preliminary Restoration | No Violations | On this day, I am conducting a preliminary restoration inspection. The site meets ORC 1509.072 (A) with vegetation established and no substantial erosion. I found the well equipped with 4.5 inch diameter casing and 2.375 inch diameter tubing with the production valves open. It is setup to be produced by means of a pumping unit with rods and a gas engine. The production facility consists of (2) 100 barrel steel tanks, (1) 1,550 gallon polymer tank, drip tank, vertical separator, and sales meter all contained within an earthen dike with livestock fencing surrounding the berm. The tank is properly identified and the production records are current. |
1351708217 | 2012-10-31 | Production Wells | No Violations | Inspection found the well drilling and stimulation process has been completed with Harmons service rig on the hole running the rods and tubing and the flow back water being hauled off the location. |
1360931902 | 2013-02-15 | Preliminary Restoration | No Violations | Inspection of the well head found the following equipment present- 4.5 production casing; 2.375 tubing and pumping rods with the production valves open and the well being produced by the means of a pumping unit. At the production fluid storage area 2-100 barrel tanks along with a mechanical separator and production gas sales meter are present in an established dike. Inspector will note that the drilling pits have been filled and the location rough graded in but the location will need seeded when the weather allows. |
1532516367 | 2018-07-25 | Production Wells | No Violations | Status check inspection found the well set up to be produced by the means of a pumping unit with a natural gas driven engine. The well is equipped with 4.5 diameter production casing and 2.375 diameter non upset tubing. There is both casing and tubing well heads present with production valves attached and in the open position. Both the tubing and casing flow lines attach into a 2 diameter production flow line near the well head. The production facility consists of 2 steel design 100 barrel fluid capacity tanks along with a 1,600 gallon plastic brine tank. There is a mechanical separator present along with a gas sales meter. The production facility has an earthen dike erected around it. Proper well identification is posted. |
