0788657064 2015-06-18 Preliminary Restoration No Violations Preliminary Restoration Status check inspection found the well location meets 1509.072 with vegetation established and erosion control in place. The well has been drilled but not stimulated. There is however 6 gas processing units on place on location to support each of the 6 wells present in which at this current time all 6 have been drilled but only 3 completed and in production with a total of 6 -210 barrel capacity production fluid tanks present in a plastic lined metal containment dike. In conversation with the newly acquired owners Gulfport Energy they intend on making changes to the well pad perimeter containment by adding functional drain valves at the discharge points along with rebuilding the well pad earthen containment berm. Preliminary Restoration passed on this day.
1032201241 2013-12-22 Drill / Deepen / Reopen No Violations Status check inspection found the current drilling depth to be 10,063 using a 8.5 drill bit in the production borehole stage of the well. The mud weight is 13.25#. A follow up on the recent oil based drilling mud spill found the company working to finish clean up and 3rd party contractors on site. Photographs were taken and the current clean up situation was discussed with my supervisor.
1175412169 2013-12-26 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found the well production borehole total drilling depth has been reached at 15,941 using a 8.5 drill bit. The recent oil based mud release clean up has been completed and the company is working on installing a fluid recovery pit below the location drain point, all surface water will be recovered from this point.
1246465250 2016-12-29 Completion Testing No Violations On this day I met with Gulfport field supervisor David Wade and reviewed a casing bond log with the log showing over 70% bond at a measured depth of 14,000 in which is the depth that the company proposes to set a 10,000 psi rated cast iron bridge plug to isolate the well bore from the depth the previous plug was set at 15,329 measured depth from surface. They are indicating this is giving them a 500 set back off the BLM property.

Inspection found several 3rd party agencies on location working to recover the fluid, Matt McGuire with HG was on site and we discussed the clean up procedures as well as future preventive action. Pictures were taken on this day.
1372165203 2013-06-25 Casing String Witnessed No Violations Status check inspection to witness the setting and cementing of 1093 of 13.375 surface pipe. Decker Drilling rig # 6 is on the hole with Baker Hughes cement services. The cement mix water was tested by the Baker Engineer and myself and showed no chlorides and a PH level of 6. There were 40 barrels of cement returns to surface and the casing was pressure tested to 670 psi upon the landing of the plug.
1386331750 2013-12-05 Drill / Deepen / Reopen No Violations HAD drilling rig 105 is now rigging up and shall begin drilling the 9.625 intermediate casing well bore. The company is switching out Blow Out Preventer stacks to a 10,000# stack and shall submit a copy of the BOP test.
1409590331 2016-12-16 Completion Testing No Violations On this day I participated in a conference call between Division staff members discussing the horizontal well bore being drilled across property that has been indicated to be the property and a mineral rights of the Bureau of Land Management (BLM). Gulfport has submitted a proposal to set (2) cast iron bridge plugs with a 500 stand off from the (BLM) property boundary. Gulfport has indicated that the cast iron bridge plugs proposed to be used to plug the wells will satisfy the (BLM). The well was drilled by a previous owner HG Energy. The well stimulation process has not been initiated.
1479822121 2016-11-21 Completion Testing No Violations Status check inspection observed that on this day crews were on site from Wehr well head installing the well head (B) sections and completion master valve assemblies and pressure testing the well head assemblies.
1482930739 2016-12-27 Completion Testing No Violations Status check inspection on the well completion workover process found a service rig on the hole owned by Hybrid drilling. The company man on site David Wade reported that a cast iron bridge plug was set at 15,329 measured depth to cover the 5.5 casing toe sleeve at 15,334. Nine Energy is standing by on location with a wireline truck to run a bond log.
1483218109 2016-12-31 Completion Testing No Violations On site to witness well owner run Cast Iron bridge plug into well, they ran a Baker 10K bridge plug to 14,045 and set it with setting tool and then pulled tubing from well bore and they plan on pressure testing plug to 9800# to confirm that it is holding and then they plan on repeating process and set second plug@14,000 and pressure test it also then they plan on resuming frack operations on said well.
1483282947 2017-01-01 Completion Testing No Violations On site to verify setting depth of CIBP and to pressure test plug integrity, Full Circle was plug tester they applied 2500 psi and then they raised pressure in 2500 psi steps until they reached 9800 psi and we monitored pressure for 15 minutes with a 75 psi loss during test period TEST PASSED and they will run second CIBP and test it also and then they will resume complexation operation.
1618661012 2013-12-16 Drill / Deepen / Reopen No Violations Inspection found the current drilling of the 1st intermediate 12.250 section at 8395.
1850109360 2013-12-11 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling progress found the current drilling depth to be 5,450 using a 12.250 hammer bit and the drilling company pulling out of the hole with the drill string for a bit change. The estimated total depth to be drilled for this intermediate well bore section is 8350.
1972066749 2017-06-27 Production Wells No Violations Status check inspection found 6 wells present on location and in production. All wells are equipped with tubing heads and the production valves are in the open position. A total of 6 gas processing units are present on location. A total of 6 production tanks are present inside a polymer lined steel containment dike area. Proper signage is posted at the well pad entrance on the day of my inspection.
