0222155089 2013-01-28 Drill / Deepen / Reopen No Violations Inspector on site for a status update found the company rigging up Dallas Morris rig #24 to begin completing the drilling process. Company records show the well was spudded on 04/02/12 and 80 of 20 conductor was set with no annular space.
0301896590 2013-10-07 Production Wells No Violations Preliminary Restoration found the wells on site all have been drilled and stimulated and the production equipment in place awaiting the gas processing transporting facility to begin purchasing the product. 6- 210 barrel storage tanks are on site in 2 tank individual containment areas, Proper well head identification is in place along with identification at the well site entry point. The site meets ORC 1509.072 regarding restoration.
0456778647 2015-06-18 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well is in production with the production valves in the open position and 3 sets of 2-210 barrel production tanks on location in a plastic lined steel containment area supporting the said well along with the 5 other producing wells on location, there is also a total of 6 gas processing units on location supporting each well on the pad. Due to the recent rainfall standing water was found present on location and tested for chlorides with Low Level Quantab strip field strip tests finding no Chlorides on the entire location over 121 parts per million on this day. Also no sheen was observed as present. Under ORC 1509.072 the Preliminary Restoration was passed on this day with vegetation established in the area affected by the initial well pad construction and erosion control in place.

The current bore hole position is just starting into the horizontal curve.

1168340280 2014-05-31 Casing String Witnessed No Violations On 05/31/2013 Decker drilling rig 10 is now on the hole and has drilled a 17.5 surface borehole to a depth of 1,085 with 1,071 of 13.375 surface pipe ran and cemented by Baker Hughes using 850 sacks of class A .25 pound per sack flake, .5% calcium and 35 barrels of cement returns to surface with the casing being pressure tested to 845 psi.
1360192490 2013-02-06 Drill / Deepen / Reopen No Violations Notification was received @ 9:07 Am from Josh Hinton from HG energy that the surface bore hole was loaded with A 500 barrel mix of brine water and other hole conditioning material. Josh indicated that did not realize their mistake until after they had completed the process of trying to retrieve the lost cutting head off of the hammer bit and stabilize the bore hole. I immediately made notification to my supervisor Rocky King and MRI 3 over the regions drilling and plugging Pat Shreve and the decision was made to allow the company to continue running the surface casing into the bore hole and then circulate the unwanted fluid out of the hole. A full description and amount of the fluids introduced into the well bore was received. Inspector will note that I was on site through out the entire running of the casing; circulation of the mud flush and cementing procedures. The drilling permit was issued 04/05/2012.
1360597888 2013-02-11 Drill / Deepen / Reopen No Violations Upon inspection the current drilling depth was found to be 1267 using a 12.250 bit. A 17.5 bit was lost at 1247 and was tagged at 1224 with the bottom of the surface set on top of the lost tool at 1140. The borehole below the surface then was cemented with 200 sacks of type 1 cement. The company allowed the cement to set and then went back to drilling and deviated the borehole around the lost bit.

1361497346 2013-02-21 Drill / Deepen / Reopen No Violations Inspector on site from 7AM until 3:30 PM to witness the cementing of 8273 of 9.625 casing in a 2 stage process with 30 % cement excess ran on both stages and the 2 stage tool set at 5448. A copy of the casing tally and cement job tickets was received.

1367283430 2013-04-29 Casing String Witnessed No Violations I received notification of the completion of the drilling process and the running and cementing of the production casing on the weekend of 04/27 & 04/28 2013. On 04/ 29/ 13 I picked up the casing and cement records and completed the casing report in the database. The company also informed me on this day that the well stimulation process would begin immediately on the 5 completed wells on this drilling pad.
1367536262 2013-05-02 Drill / Deepen / Reopen No Violations On this day whil doing a site inspection I observed the well bubbiling between the 13.375 and the 9.625 casing strings. My completion records showed that we bond logged the 9.625 casing cement job due to no cement returns with the bond log showing the cement top at 200 from surface and the bottom of the 13.375 at 1247. We will continue to monitor and access the situation.
