0436728540 | 2018-03-07 | Final Restoration | No Violations | Status check inspection to document the well was drilled and plugged back and is now operating under the permit number 3411124331-0100. |
0583273175 | 2019-01-24 | Production Wells | No Violations | On this day, I am conducting an inspection to document the SIMOPS progress for the plug and abandonment of permit number 4446. I spoke with Antero representative Barney Etcher, and he stated the SIMOPS process is being delayed due creating a standard operating procedure between departments within Antero Resources. Containment is being set in place and steel cages for the producing wells are onsite. The well in question currently has a nightcap in place and no gauges are present. I found the pad gated with signage at the entrance. There are (6) producing wells, (6) gas processing units (GPU), (4) 400-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Additionally, concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 100 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi. |
0668358091 | 2013-08-14 | Production Wells | No Violations | The said well is now in production. The tubing pressure id 3745 psi. I asked the company man on site to place proper well identification on each well head. |
0976500081 | 2015-06-17 | Preliminary Restoration | No Violations | Preliminary Restoration status check inspection found the well location has proper erosion control and vegetation established. The company has recently added additional earthen containment around the well pad perimeter. Addressing the well status the well drilling and completion process is complete with the well in production with additional gas processing equipment located on the well pad in a separate location from the producing wells, there is also a total of 8-400 barrel capacity production fluid tanks on location located inside a plastic lined metal dike area with proper well identification posted at the well head. Preliminary Restoration passed on this day. |
0982918977 | 2019-01-29 | Production Wells | No Violations | On this day, I found Antero progressing with the SimOps procedure in preparation for the plug and abandonment of permit number 4446. TekSolv is onsite installing back-pressure valves in the tubing hanger on the (6) producing wells on the pad, prior to installing the steel cages over the wellheads. Fortis Energy is also onsite installing containment and preparing to rig-up, once the SimOps procedures have been completed. I spoke with Antero Representative Barney Etcher during the inspection. |
1055457986 | 2012-05-10 | Drill / Deepen / Reopen | No Violations | Stopped by drill rig for update on upcoming surface job, consultant stated that they should be ready to cement pipe in place late Friday night or Saturday morning. |
1144138400 | 2014-03-13 | Production Wells | No Violations | Inspector Eric Conrad and myself met on site with Anteros production supervisor Daren Rader discussing several site concerns listed as follows, site entry point identification, proper site soil compaction over the recently installed production flow lines, procedures for identifying temporary storage tanks with proper liquid content identification, general procedure safety during the completion drill out process, proper mapping and identification of buried underground production lines, proper containment under all fluid vessels on site. Mr. Rader understood this was to be addressed as procedure concerns on all Antero sites in general. |
1304598486 | 2012-07-06 | Completion Testing | No Violations | Inspection revealed that frac job is complete and company is getting ready to start flow back process, currently rigging up flare stack and flow back lines to 500 bbl storage tanks. |
1339619398 | 2012-06-13 | Drill / Deepen / Reopen | No Violations | Total depth@14,686 preparing to run 5.5 casing. |
1361302138 | 2013-02-19 | Production Wells | No Violations | Inspection of the well and site found the well has been completed and is idle waiting on a gas transportation line to installed. 8- 400 barrel production tanks are present on location in a lined metal pit. A lined fresh water impoundment is located on location in a fenced in area. |
1385061534 | 2013-11-20 | Production Wells | Violations Noted | On site electrical inspection with Brad Sellers the Divisions inter agency coordinator and a inspector with the Ohio Department of Commerce- electrical inspector, On the day of inspection deficiencies were found in the grounding system on the electrical service feeding the site, We also discovered a 500 barrel tank on site used for flow back operations with no plugs in the outlets. Anteros people were on site and made the needed corrections accordingly. |
1389812434 | 2014-01-15 | Preliminary Restoration | No Violations | Status check inspection found the well in production and the boundary areas disturbed by the well pad construction have ample vegetation. 8- 400 barrel production tanks are located in a containment dike area. |
1677943755 | 2012-06-11 | Drill / Deepen / Reopen | No Violations | Total depth@14,686 conditioning well bore will run 5.5 casing on Wednesday. |
1736047510 | 2018-03-07 | Production Wells | No Violations | Status check inspection accompanied by ODNR Inspector Tom Thomas to document the well status. The well is equipped with a casing and tubing well head assembly with (2) manually operated valves on the tubing well head and one emergency shut down valve on the tubing tree production flow line. The well head casing flow line is also tied into the production flow line however the valves are in the closed position. The 13.375 and 9.625 casing well head assemblies have flowlines with valves and gauges installed to monitor annular pressure. The 13.375 casing well head pressure gauge reads (0) zero pounds pressure and the 9.625 casing well head reads 700 pounds pressure. On pad total is (6) complete wells supported by (6) gas processing units. Also on location is a production fluid facility consisting of a total of (8) steel construction tanks with a capacity of 400 barrels of fluid each, the tanks are situated inside a polymer lined steel containment dike area. |
1884522499 | 2013-10-02 | Preliminary Restoration | No Violations | Status check inspection on the well and location, On this day I spoke with Anteros safety coordinator and asked that each well head have proper identification and proper identification be placed at the entry point of the location. There are also contractors on site emptying the earthen pit and preparing to close it in. The area around the site has vegetation established and the drilling equipment has been moved off with the well currently in production. The tank battery has been erected on the site pad with 8-400 barrel production tanks present. |
2042933591 | 2018-03-07 | Production Wells | No Violations | On this day, I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a detailed report. The well is equipped with annular pressure monitoring devices. Concrete barriers surround the wellheads. The facility is gated with an electronic shutdown button on the right-hand side, prior to the pad. The front left of the pad is equipped with (8) 400 barrel steel tanks within containment and an electronic shutdown button. There are (6) gas processing units along the front right side of the pad, and a midstream facility along the front right side, as well. |
2058474054 | 2013-10-27 | Production Wells | No Violations | Follow up inspection to complaint # 4510 forwarded over from Aaron Wolfe with the Ohio EPA, The photographs sent to the EPA by Ruth Partin were reviewed by my Supervisor Rocky King and Myself and along with the EPA it was found no violations were present in the photos, The request by Ruth Partin to disclose the nature of the production equipment on location will be for site safety reasons forwarded to Anteros public relations department for answering, The EPA provided only a email address and failed to document a time or day of contact as well as any personal contact information for Ruth Partin. My on site inspection found my recent request after my last inspection on 10/02/2013 to post proper identification met, All 3 wells on location are in production and a Myers tank truck was on location withdrawing processed water with proper safety grounding and spill containment in place while loading, The Myers UIC number was also properly posted on the tank truck. |
