0932950828 2018-03-05 Production Wells No Violations On this day, I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a detailed report. The lease road is gated and an EQT midstream production facility is situated on the righthand side, prior to the well pad. There is an emergency shutdown button on the left, at the entrance. The well is equipped with annular pressure monitoring and an emergency shutdown button is located at the wellhead. There are (6) gas processing units on the left of the pad, and all are equipped with an emergency shutdown button. At the backside of the pad are (4) 400 barrel steel tanks within containment.

1297065579 2018-03-05 Production Wells No Violations Producing well status check inspection found the well head designed to produce from the tubing well head above the B-Section and casing head section. In the tubing section vertically there is a total of (2) manually operated valves then the tubing flow cross, attached to the tubing flow cross downstream is a third manual valve then a Emergency Shut Down (ESD) valve with remotely controlled switches at multiple locations on location including at the well head and gas processing units along with the production facility and entry point of the location. On location there is a total of (6) active well heads with annular pressure monitoring in place on each well head. The said well has annular pressure gauges in place with the valves in the open position. The annular pressure between the 13.375 casing and 9.625casing reads 0 psi and between the 9.625 casing and the 5.5 casing 0 psi. Addressing the well support equipment on location- at the entry point of the location is a midstream operation operated by EQT Company, on the left hand side of the well pad is a total of (6) gas processing units one each for every active well on the pad. At the left rear corner of the well pad is the production facility consisting of a total of (4) steel construction tanks each with a total fluid volume of 400 barrels capacity, the tanks are situated inside a polymer lined steel containment dike area. The well pad does have a perimeter earthen berm. Proper well identification is posted at the each well and the tank battery as well as the entry point of the lease road and well pad.
1421854426 2015-01-21 Casing String Witnessed No Violations The said well was originally spudded by HG Energy on 12/29/2013 as the #8 well with conductor and surface casing being set prior to AEU taking ownership. On 01/19/2015 notification was made by the company man on site that they have now expired 2 drill bits at a depth of 5,540 in the Oriskany formation using synthetic oil based drilling fluid and have been unsuccessful in blowing the hole dry below the Berea formation. A Verbal Variance was granted by Deputy Chief Dave Claus to set the 9.625 casing short of their proposed total depth.
1639237729 2014-05-23 Casing String Witnessed No Violations Decker drilling rig #9 drilled an open hole of 17.5 to a depth of 1085. They ran 13.75casing of 54.5lb/ft. to a depth of 1073. The float was set at 1039. They ran an auto fill insert with a cement nose guide shoe. All casing was 8 round range three. Baker Hughes was performing the cementing services. They pressure tested lines to 2250 psi. They pumped capacity and the spotted a gel spacer of 30 barrel. Then started cement of 850 sacks of Premium NE-1. With .5% of CaCl and .25lb per sack of celloflake. All cement was mixed at 15.6 lb/ft. There was good circulation with 30 barrel of cement returns. They landed the rubber wiper plug and pressure tested the casing 500 psi over for 5 minutes.
1717819958 2015-01-13 Drill / Deepen / Reopen No Violations Status check inspection found Nomac rig # 82 on location drilling with synthetic oil based 12.5# fluid and deviating at 12 degrees of inclination in the direction of 17.410 degrees azimuth around a lost tool string in the 12.250 1st intermediate well bore section at a depth of 5,540. A 17# class H kick plug was set on 01/11/15 and top of the plug tagged up on at a depth of 4,245 with their current drilling depth of 4,428 with a reported surface return cuttings of 80% cement and 20% formation the company will continue to time drill around the lost tools.
1725702044 2017-03-06 Preliminary Restoration No Violations Preliminary Restoration status check inspection accompanied by ODNR County Inspector Austin Gutridge. The area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. The lease road is stoned and in good repair. Signage is posted at the well pad entrance. Evolution Energy Service DC turbine electric powered stimulation pump fleet is on location and has stimulated 19 stages on the 5-H well. Red Dirt rentals completion master valves are in place on each of the 5 wells on location. Nine Energy completion service is on location perforating each stage of the well bores to be stimulated with 5 clusters of 10 shots each cluster. The well completion iron is 15,000 psi rated capacity. All completion equipment is located on secondary polymer containment with the well pad berm serving as the primary containment with the perimeter drain valves in the closed position.
2008096039 2017-06-27 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072 with vegetation established and erosion control in place. The lease road is in good repair and graveled from County Road 12 to the well pad. There is a midstream distribution sales point located on the South East side of the well pad near the well pad entry point, Rice Energy is the Mid Stream operator and there is a glycol dehydration unit present on the mid stream site. Situated on the well pad is a total of( 6) wells all now in production with production trees in place and the well head production valves in the open position. The wells are all equipped with annular monitoring pressure gauges. The wells are selling into gas processing units with a total of (6 )gas processing units on location one for each well. Also present on the West side of the well pad is the production fluid storage area consisting of a total of (4) steel construction 400 barrel capacity production tanks located inside a polymer lined steel containment dike area. The pumper is on site and has indicted they are shipping 2,100 barrels of production flow back fluid per every 24 hours. Identification is posted at each well and at the end of the lease road. Safety shut down points are at each well along with at the production facility and well pad entry point.
