0087552838 2014-05-07 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found the current drilling depth to be 8,014 with a projected true vertical depth of 10,869, the company will do various well bore formation surveys then plug the well back to their horizontal kick off point.
0130985333 2016-06-20 Completion Testing No Violations Status check inspection on the current well stimulation process found Halliburton Completion Service on location with 6 of the proposed 22 stages currently stimulated. Horizontal Wireline Services is on location and running into the well bore with a perforating gun and solid bridge plug with the plug to be set at 15,568 and the well to be perforated with 40 perforations shot in 5 clusters of 8 shots between the measured depths of 15,598 and 15,840 with the total well bore effected area for each stage being 300 from bridge plug to top perforation. All completion equipment is placed on polymer containment.
0182724941 2016-06-16 Completion Testing No Violations Status check inspection on the well completion process found Halliburton Completion Service on location attempting to stimulate the 1st stage of the well completion process. The company has pumped a total of 6,000 gl of a mixture of 15% & 28% HCL into the well bore and has pumped 1,500 barrels of H20 behind the HCL at a rate of 29 barrels a minute and pressure of 10,980 psi. The completion well head contractor is FMC and the completion iron is 15,000 psi rated working pressure. Horizontal Wireline Company is on location along with Roc Water flow back services. All completion support equipment is located on polymer containment and all completion operations are during daylight hours only.
1089342143 2018-01-10 Work Over Reconditioning No Violations On site status check inspection found the company preparing to secure the well heads and subsurface well bores in anticipation of bringing in a drilling rig to drill additional wells on the pad. The wellbore is currently equipped with 5.5 diameter production casing with 2.875 diameter production tubing installed into the well head set on a down hole compression set down packer. The company has indicated they will mobilize KLX Slick line wireline service and set retrievable bridge plugs inside the 2.857 tubing that is set on a compression set down hook wall packer. Once the tubing is isolated the company will pull the tubing out of the packer and pull 10 joints of tubing and then hang the remaining tubing in the well bore off of a packer up hole near surface. A (BPR) back pressure valve will be installed inside well head B section before the well head is disassembled and a R-24 night cap is installed on the well head. All production equipment will be removed and the remaining well head will be below grade level.
1397929678 2014-04-19 Casing String Witnessed No Violations The said well was spudded on 11/13/13 by Butches Rat hole and 100 of 20 conductor was set and cemented back to surface on 11/14/2013 by Formation cementing. On 04/19/2014 531 of 13.375 surface casing was ran and cemented back to surface by Halliburton Cement services with 24 sks of class A cement used with a casing shoe and sting in float collar ran with 50 barrels of gel spacer ran ahead with circulation established to surface prior to cementing with 15 barrels of good cement returns to surface and the mix water tested with chloride levels showing below 100 parts per million, the casing shall be pressure tested with the blow out preventer when it is nippled up.
1400241536 2014-05-16 Plug / Plug Back No Violations Orion Rig # circulated on 8.5open hole with 12.6 ppg mud. Then ran 2.875tubing at 7.9lb/ft at depth of 10406.07. Then a 2 change over sub to 5.5 @ 568.85 @19.5lb.ft drill pipe. and circulated on hole with fluid weight of 12.6 ppg. Halliburton is setting the bottom plug at 11068-10565with 194 sacks at 1.02 yield and 4.06 gallon per sack mix water requirement. The weight of cement slurry is 16.8 ppg. The kick plug will be set at 10500-9900. There was 252 sacks of dense cement run with a yield at .94 and a 3.46 gallons of mix water per sack. The weight of the cement slurry is 17.4 ppg. The 9.625casing is set at 6160. They are running a tuned spacer ahead and behind of every plug and it weighs 13.5lb/gallon. Good circulation was established. The Ph of the mix water was at 6 and the temperature was at 77 degrees Fahrenheit.
1457477978 2014-04-25 Drill / Deepen / Reopen No Violations Status check inspection of the drilling process found the current drilling depth to be 3,639 using a 12.250 drill bit and the company drilling with synthetic bases drilling mud.
1465483128 2016-06-09 Completion Testing No Violations Status check inspection on the well completion process found Deep Well Service tubing snubbing unit rigged up on the well. Historically the well was drilled and 18,206 of 23lb/ft 5.5 production casing was ran in the well with a toe sleeve in place at 17,948. Currently the service company ran in the hole with a 15,000 psi rated cast iron bridge plug and set it at 17,968, the tubing was pulled and the casing was pressure tested to 12,500 psi. The company the ran back in the hole and perforated the 1st stage to be stimulated between the measured depths of 17,933 to 17,706, the well was perforated in 4 stages with 10 perforations in each stage. The snubbing unit shall now pull the tubing and break down from the well.

2028913484 2016-06-02 Completion Testing No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072 with erosion control and vegetation established. Addressing the well head- The company has contractors mobilized on location installing polymer containment in preparation to complete the well stimulation process which is slated to begin within the next 14 days. Also scheduled is a tubing snubbing unit is slated to move onto the location in the next 2 days, the snubbing unit is owned by Deep Well Service and will run in the hole and set a retrievable bridge plug in order for the 5.5 production casing to be pressure tested. Halliburton pressure pumping stimulation service operations will then follow.
