0795691574 2017-09-06 Drill / Deepen / Reopen No Violations On site inspection found H&P Drilling rig 611 on location and rigged up on the hole. Weatherford Wireline Service is bond logging the previously set 9.625 casing string on the said well. The drilling rig and support equipment is placed on polymer containment with the well pad perimeter berm serving as the primary containment.
1216822774 2017-01-24 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site has earthen slips on the East and South West side fill slopes along with a slip below a dirt stockpile on the South East slope. The slip on the South West slope breeched the limits of disturbance depicted on the original as built well plans. The slips have not been repaired. The well pad is in violation of ORC 1509.072. The well was spudded on 02/28/2014 and 20 diameter conductor casing was set to 100, following and a 1st and 2nd intermediate casing string set with no production borehole being drilled. No production equipment is situated on the well pad. The drilling permit was re permitted and will expire on 04/11/2018.

1406572934 2014-07-28 Casing String Witnessed No Violations 07/28/2014 -4:30 am call out for the surface casing cement job and upon arrival it was discovered that the company was having difficulty making up the buttress thread casing connections and only had one joint of casing in the hole. 382 of 2094# J-55 pipe has been ran and cemented by Halliburton cement services using 415 sacks of Class A-2% Calcium-1/4# pound per sack flake cement with circulation established prior to cementing and 5 barrels of good cement returns to surface.
1407152004 2014-08-04 Casing String Witnessed No Violations Administrative report to document the failed 13.375 1st intermediate casing cement job. The company man on site reported on 08/03/2014 that during that approximately 100 barrels into cement displacement they encountered high displacement pressure not allowing them to complete the cement job and they were running in the hole with a drill string to drill out the cement, once the cement is drilled out they were going to bond log to find the top of cement and then perforate the casing and squeeze cement. In a follow up conversation on the morning of 08/04 14 the top of cement was tagged at 530 and they were at 580 drilling cement.
1407924528 2014-08-13 Casing String Witnessed No Violations On 08/02/14 I was informed by the company man on site that the 13.375 1st intermediate casing cement job locked up at 70 barrels into cement displacement. With the following repairs being carried out as follows- a bond log was ran showing the top of cement at 1,122 with perforations then shot between 1,000 and 1,008 with circulation then established-
1458886551 2017-10-10 Plug / Plug Back No Violations Status check inspection on the well plugging process of the 8.5 open well bore section found H&P drilling rig 611 on location. The company has recently hit a loss circulation zone while drilling the 8.5 diameter horizontal well bore at an approximat dapth of 12,500. The company set a 100 barrel class H isolation squeeze plug with the drill string setting at 10,800. They then reported the well bore was static and holding a hydrostatic colum of flid and at that poind per my request set a second isolation plug to cover the bottom of the 9.625 casing which is set at 10,080. The plug bottom was set at 10,310 and designed to be a 500 plug consisting of 37 barrels of class H cement. The rig is waiting on cmeent to cure and circulating on the hole at 9,775 with 825 of 2.875 tubing attached to the bottom of the drill string.
1470150841 2017-10-10 Plug / Plug Back No Violations On the day of my inspection, I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a detailed report. H&P FlexRig 611 has attempted drilling through the Point Pleasant and encountered a fault. Triad has opted to plug this well, and O-Tex is on-site to run cement and set the plugs. The top of the plug is set at 9,850 feet.

Upon inspection we found that H&P FlexRig 611 drilled to a depth 12,537 feet MD. They are currently fishing in the hole with the top of the fish at 11,155 feet MD
1632814656 2016-02-10 Drill / Deepen / Reopen No Violations The said well was spudded on 02/28/2014 with 100 of conductor being set. In the following months a rig owned by Alpha Hunter drilling was mobilized on site and the well was drilled to a point in the vertical section and the 9.625 casing being the last string set. The production borehole has not been drilled or the production casing set. The well is incomplete and the drilling permit is set to expire on 05/01/2016.
1785806464 2017-09-26 Plug / Plug Back No Violations Status check inspection to witness a open hole plug back of the 8.5 diameter production borehole due to a lost directional drilling assembly at a measured depth of 12,553. H&P Drilling rig 611 is rigged up on the well and the Company reportedly drilled into a fault on 09/21/2017 with total fluid circulation lost and the borehole on a vacuum a total of 2,000 barrels of 13.2 pound per gallon diesel fuel based drilling mud was lost into the formation along with an additional 700 barrels of heavyweight loss circulation fluid with no circulation regained. The drill string would initially rotate and was pulled back up hole approximately 70 before becoming stuck. At that point a free point wireline tool was ran and the drill string shot off at 11,155 and recovered from that point. On this day I witnessed O-Tex cement service pump a 35 barrel 500 calculated hole volume 17.4 pound per gallon class H cement plug at a measured depth of 11,131 with 15 barrels of 15 pound per gallon space ran ahead of the plug and then the 35 barrel plug, then finally displaced by 5 barrels of gel spacer followed by 168 barrels of drilling mud.
