The production facility consists of (2) 100 barrel steel tanks, (2) 50 barrel polymer tanks, and a vertical separator all contained within an earthen dike. The tank is properly identified and the production records are current. Mr. Christman also stated that the well is currently providing domestic gas. |

1442936012 | 2015-09-22 | Production Wells | Violations Noted | On 09/21/2015 I visited the site and notified the producer Dean Christman and discussed the need to empty the 300 barrel horizontal tank and remove it from the well site, also discussed was the need to remediate an area below the 300 barrel tank where discoloration and staining is present, the producer has indicated it is where the 3 valve is leaking. No standing fluid was noted as present on 09/21/2015. On 09/22/2015 I re-visited the site once again to find standing fluid in small puddles below the 300 barrel tank, the fluid was Titrate tested for chlorides with low level strip exceeding the maximum readings of 611 parts per million and reading 1139 parts per million on a high level Quantab strip test, also noted was a visible sheen on the fluid surface and visible discoloration of the soil with a total measured area of 23 in width and 60 in length. The well owner was once again contacted on this day 09/22/2015 at 11:15 am and advised that more standing fluid was present today than on my visit the previous day and that the well was not a Mississippian Exempt well and the affected area discoloration and staining along with the standing fluid was consistent with direct discharge from a tank and not a leaking valve. With the following instructions given -(1) Stop the surface discharge of fluid and remediate the affected area. (2) Remove the 300 barrel temporary storage production tank. (3) Complete the site restoration and establish proper erosion control and attempt to establish vegetation (seed & mulch). Complete all tasks within 10 days.
