1145876469 2019-04-11 Production Wells No Violations On this day, I am accompanied by well owner Dean Christman. I found the well equipped with 4.5 inch diameter casing and 1.5 inch diameter tubing with the production valves open. It is setup to be produced by means of a pumping unit with rods and an electric motor. There appears to be a slight stuffing box issue, and Mr. Christman will be addressing it. The production facility consists of (2) 100 barrel steel tanks, (2) 50 barrel polymer tanks, and a vertical separator all contained within an earthen dike. The tank is properly identified and the production records are current. Mr. Christman also stated that the well is currently providing domestic gas.
1331324439 2015-06-02 Preliminary Restoration No Violations Status check inspection on the Preliminary Restoration found the well is in production with the following equipment present at the well head-4.5 production casing -1.5 tubing and pumping rods with the production valves in the open position and the well set up to be produced by the means of a pumping unit. At the production fluid storage area 2 -100 barrel production tanks are present along with a 1500 gallon plastic brine storage tank with proper well identification being posted. Addressing the restoration the drilling pits have been filled but no attempt has been made to establish vegetation, Dean Christman the well owner was contacted on this day and informed the restoration needed completed.
1402020426 2014-06-05 Casing String Witnessed No Violations The said well was spudded on 06/05/14 by Monroe Drilling with 107 of 26# -9.625 conductor being set in a 12 x 117 borehole and cemented by Formation Cementing using 50 sacks of class A neet cement with 200# of gel ran ahead and 6.2 barrels to displacement.

1442849293 2015-09-21 Preliminary Restoration No Violations Preliminary Restoration follow up inspection found the producer has attempted to complete the restoration since my last visit. The location does not have adequate vegetation and erosion control in place. There is a 400 barrel horizontal tank on location that is leaking. The well owner Dean Christman was notified at 11:50 am on this day and informed to compete the restoration and fix the leaking tank valve, He was also informed that the tank would need to be in a dike or emptied- removed and taken out of service.
1442936012 2015-09-22 Production Wells Violations Noted On 09/21/2015 I visited the site and notified the producer Dean Christman and discussed the need to empty the 300 barrel horizontal tank and remove it from the well site, also discussed was the need to remediate an area below the 300 barrel tank where discoloration and staining is present, the producer has indicated it is where the 3 valve is leaking. No standing fluid was noted as present on 09/21/2015. On 09/22/2015 I re-visited the site once again to find standing fluid in small puddles below the 300 barrel tank, the fluid was Titrate tested for chlorides with low level strip exceeding the maximum readings of 611 parts per million and reading 1139 parts per million on a high level Quantab strip test, also noted was a visible sheen on the fluid surface and visible discoloration of the soil with a total measured area of 23 in width and 60 in length. The well owner was once again contacted on this day 09/22/2015 at 11:15 am and advised that more standing fluid was present today than on my visit the previous day and that the well was not a Mississippian Exempt well and the affected area discoloration and staining along with the standing fluid was consistent with direct discharge from a tank and not a leaking valve. With the following instructions given -(1) Stop the surface discharge of fluid and remediate the affected area. (2) Remove the 300 barrel temporary storage production tank. (3) Complete the site restoration and establish proper erosion control and attempt to establish vegetation (seed & mulch). Complete all tasks within 10 days.
