0069651533 2018-03-06 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction meets ORC 1509.072 with vegetation and erosion control in place. Addressing the well status- the well drilling -stimulation - completion and initial flow back process has been completed. The well is in production and is being produce off the 5.5 diameter production casing through the master valve and flow stack the flow stack consists of a primary master valve and 2 production valves along with a emergency shut down valve. The emergency shut down valve has several safety shut down buttons place on location and can also be shut down remotely through the SCADA computer system. Annular pressure gauges is present and the company is monitoring and recording all annular pressures. At the rear of the well pad is (4) gas processing units and on the right (West) side of the well pad is the production facility consisting of (4) steel construction 400 barrel capacity tanks situated inside a polymer lined steel containment dike area. The entire well pad perimeter is fenced and gated with signage posted at the entrance point and on each well head.

On this day my inspection found the well drilling and completion phases have been completed and contractors are on location installing 2-400 barrel production tanks and the remainder production support equipment. The company site manager indicated the construction off the production equipment should be complete in the next 14 days. I shall follow up on the Preliminary Restoration at a later date.

There are (4) producing wells , (4) gas processing units, (4) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 0 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi. The production records are current.
1204646361 2014-10-27 Completion Testing No Violations Status check inspection of the completion process found Mercer Well service work over rig on location with the completion plugs being drilled out of the hole and a compression set down packer in place on 2.875 production tubing at a depth of 10,250. While setting the packer the tubing string unscrewed at 2000 above the packer and the rig is currently attempting to get over the top of the tubing string in the hole with a overshot fishing tool on tubing, once they have ahold of the tubing the string will be released off of the top of the packer and pulled out of the hole, corrective repairs will be made and tubing ran back in and stung back into the packer and re pressure tested to insure annular isolation in the vertical section.

1398977656 2014-05-01 Casing String Witnessed No Violations The said well was spudded on 04/30/2014 by Well Service Group, Inc top hole rig with 100 of 20 .375 wall conductor being set inside a 30 borehole. On 05/01/2014 Allied Cement Services Co cemented the conductor using 350 sacks of class A-2% calcium .25 pound per sack flake cement, the mix water was tested and showed no chloride levels above 100 parts per million.

1403635979 2014-06-24 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling status found Orion Drilling rig on the hole drilling the 8.5 vertical strata test pilot hole at a depth of 6,371 using oil based drilling mud and making 18 of hole pre hour in the Onondaga formation. The company man estimated 8 to 10 days of drilling and well bore surveys before they are ready to plug back and begin the horizontal section.
1404774249 2014-07-06 Plug / Plug Back No Violations Inspection revealed that drilling contractor had reached proposed total depth on vertical section of well bore and they were planning on plug back procedure.

Note: Set 1700 plug and they plan on retesting BOP assembly before drilling on plug to attempt to start lateral section of hole.

1404811744 2014-07-07 Drill / Deepen / Reopen No Violations Status check inspection of the drilling progress found the current drilling depth to be 9,283 and the company just starting to begin their horizontal curve deviation off of the recently set vertical test plug-back and kick plug. The company has containment established on the drill site but no permiter containment erected.
1410311293 2014-09-09 Completion Testing No Violations Pre stimulation location status check inspection found Producers Completion Service rigging up and Horizontal Wire line service moving in on location. A review of the well site containment found the company (Stat Oil) has had 3rd party contractors erect a primary containment boundary around the well location along with multiple individual working area containments on the site pad with the 3 fluid catch basin drain outlets in the closed position. Discussion was made with the company men on site in regards to the on site storage quantities of well support hazardous materials. It was also pointed out that several fluid storage tanks did not have the grounding wires properly connected. MSDS sheets are posted at 4 different locations with the United Nation identification numbers included. The stimulation process is slated to begin in the next 24 hours and a metal open top poly lined fresh water container has been erected on the well site with a current volume of 22,000 barrels of fresh water. District Supervisor Brent Bear along with Division Engineers Andy Thomas and Anthony Carson were present on site on this day also.
1445964824 2015-10-27 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well now in line and producing with the following support equipment located on location - a single gas processing unit along with a gas dehydration unit, (2) 400-barrel capacity production fluid storage tanks are located in a polymer lined metallic containment area. The well pad is encased in a fenced in location and has a primary perimeter containment dike to control surface fluid discharge. Addressing the preliminary restoration- It was observed that the area depicted as to be disturbed during the well pad construction has proper vegetation and erosion control established.
1461666648 2014-06-18 Drill / Deepen / Reopen No Violations Status check inspection on the drilling progress found the current drilling depth to be 4,321 in the 12.250 intermediate well bore stage with the company drilling on oil based driling fluid with a proposed total drilling depth for this stage of 6,100.
1465811753 2014-09-13 Completion Testing No Violations Status check inspection on the stimulation process found Producers Completion Service rigged up on the hole and it was reported that during the initial attempt to open the production casing toe sleeve difficulty was encountered with a total pressure of 10,500 psi used to finally open the toe sleeve to allow the wireline tools to be pumped down hole. During the running of the wireline tools by means of hydraulic pumping a well bore restriction was encountered at 12,000 and the 10000 # bridge plug was set at that depth to release the stuck tool. Producers Services are being rigged down and coil tubing is being brought in to drill out the plug.
1486562955 2017-02-08 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well drilling and completion process final and the well in production. The well is equipped with a production tree with a inline emergency shut down valve that can be controlled from multiple point on the well pad including the main entrance gate. The well is being produced into a gas processing unit and transferred off site through a main sales line. There is a total of (4) production tanks on the West side of well pad with a total capacity of 400 barrels each. The tanks are located inside a polymer lined steel containment area. There is a total of 4 active wells on the pad. Addressing the restoration, The area affected by the well pad construction has been restored with vegetation and erosion in established. The well pad perimeter drains are being monitored and tested for chlorides prior to releasing any surface water created on the pad from weather. No drilling pits were excavated during the well drilling and the entrance road is in good repair and accessible, the well pad perimeter is fenced with a primary gate in place and signage is posted at the well pad entrance.
