0305822723 2015-09-02 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well stimulation & completion process completed and the well now in production with a gas processing unit on location supporting the well. At the production fluid storage facility a total of 4-400 barrel production tanks are present inside a polymer lined metal dike area. There is currently a total of 3 active wells on this pad. Addressing the restoration- adequate vegetation and erosion control is in place in the affected area by the well site construction.
0798544127 2014-07-29 Plug / Plug Back No Violations On 07/29/14 the company man called at 2:00 AM and informed me that they were ahead of their anticipated plugging time and they had casing in the hole with 1,100 of 3.5 tubing hanging below putting them at 10,050 approximately and they were ready to set their bottom plug, permission was granted to set the plug and the next plug above it with a total strata test depth being drilled at 10,100 and 2 plugs to be set using class H cement and Stingray cement services with the bottom plug being set from 10,100 to 9,300 and the next plug to be set from 9,182 to 8,582.

There are (3) wells setup to be produced by a plunger lift system, (1) gas processing unit, (4) 400-barrel steel production tanks within polymer-lined steel containment. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 480 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi.
