All wells have proper identification posted. There is also a total of (4) gas processing units on location supporting each of the wells. At the production fluid storage area there is a total of (4) steel construction 400 barrel capacity steel tanks present, all situated inside a polymer lined steel containment dike area with identification posted at the production facility and well pad entrance also. There is a steel gate at the well pad entrance with master control shut off buttons at the well pad entrance and 2 other points on the well pad including the well heads and gas processing units.

Addressing the preliminary restoration it was observed that there is adequate vegetation established within the boundaries of the limits of disturbance. Proper erosion control has been maintained. There are (4) producing wells equipped with a plunger lift system, (4) gas processing units, (4) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 0 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 380 psi. The production records are current.
1402638820 2014-06-13 Casing String Witnessed No Violations The said well was spudded on 06/12/2014 by Rocky Mountain Drillings top hole rig with 112 of 26 conductor being set on 06/13/2014 inside a 32 borehole with Stingray cement services cementing the conductor with 238 sacks of class A cement with good cement returns to surface.
1407460907 2014-08-07 Casing String Witnessed No Violations On08/07/14 Patterson rig #538 is now rigged up on the hole and has drilled the 24 surface borehole to 334 and ran 300 of 18.625 87.5# casing with O-Tex cement service on location to cement the casing back to surface with 470 sacks of class H cement with 30% open hole cement excess ran and 7 barrels of good cement returns to surface, the cement mix water was from a municipal source and verified good by the cement company.

The tubing flow cross section has been removed to install the plunger lift system. Annular pressure monitoring is in place. Also on location is the production facility consisting of 4 steel construction tanks capable of holding 400 barrels of fluid each. The tanks are situated inside a polymer lined steel containment dike area.

Chloride strip tests were administered in the standing water on location generated from the recent rainfall with no unacceptable levels being found, the company is also testing all fluids for chlorides.
