0182107316 2015-07-22 Completion Testing No Violations Status check inspection on the well drilling process found the well drilling and stimulation process now complete with 4 wells on location awaiting the workover rig to drill out the completion plugs and run the production tubing. All completion work is being conducted on 2 levels of containment. The company has contractors on location finishing the installation of the 4 gas processing units and well production support equipment.
0899368466 2019-04-10 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (4) producing wells equipped with a plunger lift system, (4) gas processing units, (4) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 200 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 300 psi. The production records are current.

1414772653 2014-10-31 Casing String Witnessed No Violations Status check inspection to witness the setting of the 13.378 1st intermediate well bore casing string. Nomac rig 311 is on the hole and has drilled a 17.5 1st intermediate well bore to a depth of 2,325 and is currently landing the 13.375. Halliburton Cement service is on location rigging up in preparation to cement the casing.
1415576542 2014-11-09 Casing String Witnessed No Violations Status check inspection on the 2nd intermediate casing cement job. At 1:06 pm the company man on site informed me that during the cement displacement of the 1st of 2 stages to be cemented the circulating pressure increased after Halliburton turned displacement over to the rig pumps and they were unable to maintain circulation. I responded and a cement temperature and bond log was ran showing no annular cement. Halliburton cement service then attempted to circulate and was successful in circulation the well bore. The 2nd stage cement job was then conducted.
1415748671 2014-11-11 Drill / Deepen / Reopen No Violations Administrative inspection to document notification received on this day from Gulfport employee Hal Miller at 17:03 pm in regards to a synthetic oil based drilling mud release due to a high pressure line failure. Mr. Miller indicated a high pressure line ruptured in the drilling rig back yard area at approximately 16:30 pm discharging approximately 10 barrels of drilling fluid mostly on the drilling rig and support equipment. The pumps were immediately shut off and clean up initiated. Mr. Miller indicated a small amount of fluid left the primary containment in the form of a mist, this product was being recovered by scraping up the affected gravel area. I notified ODNR Regional Supervisor Rocky King at 18:28 pm.
1415825891 2014-11-12 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process and to follow up on the reported oil based drilling fluid discharge on 11/11/14. Nomac Drilling rig 311 is on the hole drilling the 8.5 production borehole in the horizontal curve well bore section at a current horizontal depth of 9,261with a directional azimuth of 152 degrees. A site review and investigation of the oil based drilling fluid found a fluid transfer hose had failed and emitted fluid onto the drilling rig and support equipment, all equipment has been pressure washed and the accumulated fluid recovered in the amount of 60 barrels. 4 containers of gravel were scrapped up and hauled to a EPA approved landfill. No violations were issued and the clean up found to be satisfactory on this day.

1727300791 2015-05-11 Preliminary Restoration No Violations Preliminary Restoration Status check inspection and well completion process update found a total of 4 wells have now been drilled on the Campbell well pad with all wells secured and awaiting a service rig to complete the toe prep clean out post-stimulation run. Upon completion the stimulation process will take place. The company has installed all well production flow lines and a tank battery has been constructed on location with a total of 4-400 barrel production tanks present. Upon review of the well pad perimeter it was observed that vegetation is established and no erosion issues noted as observed. The company has personnel in place monitoring any water that accumulates on location before discharging any fluid off site. Preliminary Restoration passed on this day.
1747861879 2014-11-07 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found Nomac Drilling rig 311 on location rig up on the hole and the 12.250 2nd intermediate well bore total drilling depth recently reached at 8,611 with the rig conditioning the borehole preparing to pull the drill string and run casing. The well bore True Vertical Depth is 8,578 and there is a 9 degree directional nudge @ 5000.
