1221488105 2015-09-02 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well stimulation & completion process completed and the well now in production with a gas processing unit on location supporting the well. At the production fluid storage facility a total of 6-400 barrel production tanks are present inside a polymer lined metal dike area. There is currently a total of 5 active wells on this pad. Addressing the restoration- adequate vegetation and erosion control is in place in the affected area by the well site construction.
1415572099 2014-11-09 Casing String Witnessed No Violations 11/09/14 Sidewinder rig 209 is now on the hole and has drilled a 17.5 x 355 surface bore hole and ran 355 of 13.375 casing with a float shoe and float collar ran. Stingray cement service is on location to cement the casing with 462 sacks of Class A-2% calcium .25 pps flake with 25 barrels of 6% gel ran ahead and 10 barrels of good cement returns to surface and the casing pressure tested to 500 psi over cement lift pressure upon the landing of the plug.

2105809168 2019-03-11 Production Wells No Violations On this day, I found the pad gated with signage at the entrance, and an Emergency Shutdown (ESD) prior to the gate. There are (5) wells setup to be produced by a plunger lift system, (5) vertical separators, and (6) 400-barrel steel production tanks within polymer-lined steel containment. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 575 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi. I spoke with Gulfport Energy Representative Steve Habig onsite.
