0744881122 2014-09-07 Casing String Witnessed No Violations I am onsite to witness the surface casing cementing operations. Sidewinder Drilling rig 121 drilled 357 of 17.5 hole and ran 357 of 13.375 casing. Stingray Cementing LLC cemented the casing in place with 474 sacks of class A cement with 2% CaCl and .25 pounds per sack of Celluseal. I witnessed Stingray verify the cement weight at 15.7 pounds per gallon with pressurized cement scales. I witnessed twelve barrels of cement returns to surface. The casing was pressured up to 1112 psi and lost only one psi. Circulation was maintained through out the job.
0796072290 2019-03-11 Production Wells No Violations On this day, I found the pad gated with signage at the entrance, and an Emergency Shutdown (ESD) prior to the gate. There are (5) wells setup to be produced by a plunger lift system, (5) vertical separators, and (6) 400-barrel steel production tanks within polymer-lined steel containment. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 0 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi. I spoke with Gulfport Energy Representative Steve Habig onsite.
0858556991 2015-09-02 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well stimulation & completion process completed and the well now in production with a gas processing unit on location supporting the well. At the production fluid storage facility a total of 6-400 barrel production tanks are present inside a polymer lined metal dike area. There is currently a total of 5 active wells on this pad. Addressing the restoration- adequate vegetation and erosion control is in place in the affected area by the well site construction.

Sidewinder rig 121 is on location and skidding to the # 3-C well.
1424883975 2015-02-25 Completion Testing No Violations Status check inspection on the well stimulation process found Stingray Pressure Pumping service on location completing the 18th stage to be stimulated with the annular casing pressure being monitored electronically during the stimulation process, all pressure iron being used is 15,000 psi working pressure and 15 tractor trailer pumping units are in use on the well pad along with 2 mixing blenders. Nine Energy wireline service is on location and the well bore has had a 15,000 psi rated completion plug set at a depth of 13,358 on this stage along with 45 perforations in the casing shot in 5 clusters of 9 shot bursts between the depths of 13,157 and 13,338. All equipment is located on primary containment with the well pad perimeter containment berm being the secondary containment with the outlet valves in the closed position during stimulation operations per company policy.
