1408032912 2014-08-14 Surface Facility Construction No Violations On 08/06/14 during a status check inspection on the well site pad construction found the company excavating the site had pushed topsoil and vegetation(trees) outside the limits of disturbance boundaries and no perimeter silt fence had been erected, aryl Davis with CESO the design engineering firm was on site and a stop work order was given and the problems noted were to be addressed. Upon inspection on 08/14/2014 it was observed that 3 excavators were on site pulling the top soil back into the limits of disturbance and removing the vegetation. The new operators AEU has required all vegetation be ground up and removed from site. I also understand that the well pad initial plans have been altered and I have requested a new set of plans from Gary Davis with CESO and informed Mr. Davis that a new pre-site inspection would be needed before approval to construct the pad would be given.
1984735321 2014-07-10 Drill / Deepen / Reopen No Violations On the date of inspection I accompanied ODNR Inspector Bill Stolarik and ODNR District Supervisor Brent Bear to perform a pre-site inspection for an upcoming oil & gas well pad. Refer to Bill Stolariks report for further details.
