0044219529 2015-08-11 Preliminary Restoration No Violations Prelimminary Restoration status check inspection found the area affected deplicted in the pre site construction has erosion control and vegetation in place. There are a total of (4) wells on this pad all named the Howell. Addressing the status of the wells all 4 wells are capable of production with a total of 4 gas processing units in place on locatin with one each supporting each well. Near the entry point of the location located on the well pad the production fluid storage facility is located with a total of 4 -400 barrel production tanks located in a poly lined steel containment dike area. Preliminary Restoration meets 1509.072 and is passed on this day.
0183420362 2014-11-19 Drill / Deepen / Reopen No Violations Status check inspection of the current drilling process found Patterson Drilling rig 538 on the location with the 12.250 2nd intermediate well bore total drilling depth being reached at 9,231, the company is preparing to pull out of the hole with the drill string in preparation of running the 9.625 2nd intermediate casing string. The well bore True Vertical Depth is 8,989.
0600706399 2014-10-24 Casing String Witnessed No Violations The said well was spudded on 10/23/14 by Express Energys top hole rig with a 105 x 32 conductor borehole being drilled. Stingray Cement service was on location and cemented the 26 conductor pipe using 273 sacks of class A cement with circulation established prior to cementing and good cement returns to surface with the cellar being sealed with the cement returns.
0961777944 2014-11-12 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling process found the Patterson rig 538 on the hole rigging up the Blow Out Preventer on the 13.375 1st intermediate casing string. The 13.375 casing string was ran and set to a depth of 2,412. O-Tex cement service was on location to cement the casing using a lead and tail cement mix Premium cement mix with 30% open hole cement excess ran and 63.5 bbl of cement returns to surface.
0972393578 2014-11-25 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling process found the current 9.625 casing cement remediation and work over review in process with the 9.625 bottom section cement job which was initially squeezed through perforations tagged at a internal casing depth of 8,152with a calculated job displacement depth of 8,350. The company is currently drilling out the cement with KCL fluid at a depth of 8,409 and experiencing drill bit cutting problems. Once the cement is drilled out over the perforations a casing integrity pressure test will be done before drilling out the casing float collar and shoe, the company has been instructed to the bond log in which will be with a Cement Isolation Scanner.
1415395901 2014-11-07 Casing String Witnessed No Violations 11/07/14 Patterson rig 538 is now on the hole and has drilled a 22 x 345 surface borehole and set 345 of 18.625 surface casing with a float shoe and collar ran along with proper centralizer placement demonstrated. O-Tex cement service is on location to cement the casing using 530 sacks of Class A-2% calcium-.25 pps flake with 30 barrels of gel and 25# of flake ran ahead, the cement displacement shall be 95 barrels of fresh water and the casing was pressure tested to 500 psi over lift pressure upon the landing of the cement plug with 5 barrels of cement returns reported.
1416716627 2014-11-22 Casing String Witnessed No Violations Gulfport Energy had Patterson rig 538 drill a 12.25open hole to a depth of 9231. They ran 9.625casing at 47lb per foot to a measured depth of 9117. They ran a 9.625shoe and a 9.625float collar. They ran 205 total sacks of casing and 57 centralizers. They ran a Weatherford two- stage tool at depth of 5034. This tool was set at 2400 psi to engage packing element. The pipe had gotten cave in at 5200,5100 and 4800. This was noted from a free point locator. The casing was 110 high from total depth. They had just gotten into 37of the Reedsville Shale. I spoke with Mike Gialousis and the plan was to Pump and break down and pump cement of 150 barrels and displace and leave 40 barrels in pipe. I got on location and Otex was pumping on 9.625casing and started from loading casing with 8.25 barrels. Then they went to 1500 psi. They would pump up at 300 psi increments. The top pressure was at 4500 psi. This would mean that the shoe was plugged up. They thought that the packer would be set. Cased Hole solutions set the 7 holes of 3/8 from the top of 9032-9036. The Otex Company started pumping on casing and the well circulated. The Superintendent called Oklahoma and got the OK to pump the cement as one stage. The rig hooked on to casing and was pumping bottoms up twice. Otex then mixed 150 barrels of cement and displaced down leaving 40 barrel in the pipe. They then for the following 6 hours performed a hesitation squeeze on well. I left location and did not witness this or the top stage. But I required a bond log be ran after drilling out and pressure testing the pipe.
1417020434 2014-11-26 Drill / Deepen / Reopen No Violations On 11/21/14 I was notified by the company man on site that the 9.625 casing had gotten stuck while running it in the hole with a current casing shoe depth of 9,097 and total 12.250 well bore depth of 9,231. Inspector Pat Shreve responded and witness them perforate the casing above the 9.625 float collar at 9,033 to 9,037 with circulation then established and the bottom section 9.625 cement job below the DV tool then carried out successfully with the DV tool set at a depth of 5,060. On 11/26/14 I was on site and Patterson Rig 538 was drilling out the cement in the 9.625 casing at a depth of 8,984, the float collar shall then be drilled out and the casing bond logged.
1736886093 2014-11-24 Drill / Deepen / Reopen No Violations Status check follow up inspection on the recent 9.625 2 stage cement job remedial work. Perforations were shot in the casing from 9,033 to 9,037 in order to establish circulation prior to cementing due to the casing shoe track being restricted. The company has drilled out the 2 stage DV tool and shall drill out the cement over the perforations and pressure test the casing prior to drilling out the float collar and guide shoe, a cement bond log will also be ran.
1948474693 2019-05-28 Production Wells No Violations On this day, I found the pad fenced and gated with signage at the entrance. An earthen berm surrounds the perimeter of the pad. There are (4) producing wells equipped with a plunger lift system, (4) gas processing units (GPU) with sand traps, and (4) 400-barrel and (1) 50-barrel steel production tanks within polymer lined steel containment. An Emergency Shutdown (ESD) button is present at the production facility, wellheads, and entrance. Additionally, concrete barriers surround the perimeter of the wellheads and GPUs. Identification and annular pressure monitoring devices are present at the wellheads. The intermediate casing has a pressure of 480 pounds per square inch (psi), and the surface casing has a pressure of 0 psi. It will be noted that slight erosion has begun at a stockpile on the east side of the lease road, just prior to the pad. I spoke with ODNR Engineering Technician Andy Thomas to notify him.
