Once total depth is reached the Company has a 3rd party contractor know as Gyro Survey Company on location to conduct a wellbore deviation survey to indicated the well bore position due the initial well bore being lost and plugged back and the company deviating around the lost tools in the 1st well bore.

1230440628 2017-07-17 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well drilling and completion process now complete and the well in line and producing. There is a gas processing unit on location to support the well along with 8 steel 400 barrel capacity production tanks located inside a polymer lined steel containment dike area. The well site meets ORC1590.072.
1284531473 2015-08-05 Casing String Witnessed No Violations On site from 2:00 am to 4:00am initially to witness the 13.375 surface cement job with Patterson Drilling rig # 343 now on location and has drilled a 17.5 surface borehole to a depth of 1,390 and ran 1,367 of 13.375 J-55 casing with the company not initially ready due to hole conditions not allowing the casing to go to bottom and the last 4 joints having to be circulated in with fluid. I returned to the site at 8:00 am to find CalFrac Cement Service on location to cement the casing using 1,391 sacks of Class A cement with circulation being established upon my arrival. During the initial cement job the contractor CalFrac initially had an equipment failure 24 barrels into the initial cement mixture causing the job to be aborted and the cement to be pumped out of the hole. I once again left location at 11:00 am and the company man will contact me once the equipment issue is resolved and the job shall re-commence. I once again was notified at 16:25 pm that a back up pump truck had arrived and was in place to begin the job, I arrived at 17:30pm to witness the cement job with good circulation established throughout the job with gel spacer but no cement returns to surface with the casing pressure tested to 500 psi over lift pressure upon the landing of the plug. The annular side was evaluated and the cement top tagged at 35 and then topped off from surface.
1332635953 2019-01-14 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (8) producing wells, (8) gas processing units within steel containment, (8) 400-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present. Additionally, concrete Jersey barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 50 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi.

1438154385 2015-07-29 Plug / Plug Back No Violations On site inspection to witness the plugging back of the 17.5 surface hole due to the company losing their bottom hole drilling assembly with 148 of tooling left in the hole at a depth of 805. CalFrac well service set the plug using 883 sacks of class A cement through the 5 drill string set at 185. The company shall attempt to deviate around the lost tools to correctly set the permitted surface casing and continue drilling the well. Patterson Drilling rig 343 is on the hole and there is multiple live completed wells on the pad.
1438265724 2015-07-30 Drill / Deepen / Reopen No Violations Status check inspection on the well drilling process found Patterson Drilling the 17.5 surface borehole. The initial well bore vertical section was lost at 805 and plugged back to the conductor pipe. The rig is currently attempting to deviate the well bore at a depth of 191 at a inclination of 1 degree in every 100 in the directional azimuth of 75 degrees. It was observed that there was fluid present outside the primary working containment the rig and support equipment was located on. The fluid was tested for chlorides using low and high level Quantab strip tests with levels found to exceed both the high level at 6,171 parts per million and 611 parts per million on the low level strip tests. The issue was addressed with the on duty company man Steve Durment and the on duty safety representative Chris Maron with the following orders given to address the workers to conduct their operations in such a way as not to allow no product to be spilled on the location and allowed to migrate off containment. Continue to recover the product seeping from under the containment. Photos were taken of the affected areas on this day.

Allied Cement services is on location to cement the casing using 333 sacks of class A-2% calcium .25 pps flake cement with 100% open hole cement excess ran and the cement mix water tested for chlorides with a Quantab test strip showing 140 parts per million and a the PH showed 7.Cement displacement was 56 barrels with good cement return to surface witnessed.
