1444751813 2015-10-13 Casing String Witnessed No Violations 10/13/2015-H&P Drilling rig 628 is now on the hole and has drilled a 17.5 surface hole to a depth of 1,410 and ran 13.375 54.5# J-55 casing to a depth of 1,402. Stingray cement service is on location to cement the casing using 1,307 sacks of class H cement with 25 barrels of 6% bentonite gel spacer ran ahead along with circulation established prior to cementing, the casing was pressure tested to 500 psi over lift pressure upon the landing of the cement plug with good cement returns witnessed to surface.

1446597546 2015-11-03 Drill / Deepen / Reopen No Violations Status check inspection to document the final drilling process in which was completed on 10/30/2015 by H&P Drilling rig # 628 by drilling the 8.5 production borehole to a depth of 19,265 and running 19,263 of 5.5 20# P-110 Casing with a float shoe and collar ran along with proper centralizer placement being demonstrated. Stingray Cement Service cemented the casing using a single slurry of 3,620 sacks of class H cement.
1481577456 2016-12-12 Completion Testing No Violations On site status check inspection on the well stimulation process- currently the company has a service rig contractor on the hole owned by Hybrid Services. The rig is attempting to recover a lost completion tools in the 5.5 diameter casing at measured depth of 19,160. The well bore is static and NOV services is providing well head surface control. Nine Energy Services is on location standing by to disarm the wireline guns once recovered.
1520275141 2018-03-05 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. The well pad entrance is gated with signage posted. The well head is equipped with a multi bowl assembly and is set up to be produced from the tubing head well head section with (2) master control valves vertically above the tubing head and a flow cross above the upper valve. Attached to the flow cross is another master valve and then an emergency shut down. The emergency shut down valve has multiple control points on location identified as being at the well head the gas processing unit and production facility with a emergency shut down button at the location entry point also. Annular pressure monitoring gauges are present on both the 13.375casing and 9.625 casing well head section with 40 pounds per square inch (psi) between the 13.375 diameter casing and 9.625 diameter casing. 1,600 psi between the 9.625 diameter casing and the 5.5 diameter casing. The production tubing valves are open and no flow lines are attached the 5.5 diameter casing well head section. On the left (West) side of the well pad is (4) gas processing units and the production fluid storage area consisting of (4) steel tanks capable of holding 400 barrels fluid each. The tanks are situated inside a polymer lined steel containment dike area. The well pad perimeter earthen berm is intact and in good repair. The lease road and well pad has a gravel surface and is also in good repair.
1593471612 2018-03-05 Production Wells No Violations On this day, I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a detailed report. The lease road is gated with an emergency shutdown button on the left, at the entrance. The well is equipped with annular pressure monitoring and an emergency shutdown button is located at the wellhead. There are (4) gas processing units on the left of the pad, and all are equipped with an emergency shutdown button. At the backside of the pad are (4) 400 barrel steel tanks within containment. There is also a midstream processing facility at the back righthand side of the pad.

Allied Cement Service was on location to cement the casing using 270 sacks of Class A 2% calcium .125 pps flake -50% open hole excess cement with circulation established prior to cementing and good cement returns to surface.
