The said well has a casing and tubing well head in place and is equipped with tubing head valves and B section cross valves. There is a Hydraulic operated electrically controlled emergency valve on the tubing head output flow line that has shut down points at the well head and production facility along with at the well pad entry point. Also on location downstream from the well head is a sand trap and line heater and then a mechanical separator. The mechanical separator sends the processed fluid to the production facility that consists of 4 steel construction 400 barrel capacity production tanks located inside a polymer lined steel containment area. There is also 2 dehydration units on location that service the 6 total line heaters on the pad. |

Upon inspection it was discovered the product was released when an employee closed the well bore discharge flow line valves while pressure was being exerted on the well bore causing a 150 psi rated plastic cap to fail on the outlet line (Elk Horn). The company is recovering the product by pressure washing the residue and using a vacuum recovery system. All product was contained on plastic containment and to the immediate working area. |
