0280687019 2017-03-28 Production Wells No Violations On the day of inspection I witnessed the well was online with the valves in the open position. Gauges present read 5550 pounds per square inches on the casing and 4555 pounds per square inches on the tubing with annular monitoring in place. Production equipment included a sand trap, a line heater and a 2- stage vertical separator. Also present included 4-400 barrel tanks within containment, 2 filter separators, 2 vertical scrubbers, 2 heater treaters and a gas combustor all shared with 34111246100000 Pittman 3H, 34111245910000 Holliday A 3H, 34111245960000 Holliday B 7H and 34111246090000 Circosta 1H.
0663767519 2017-01-05 Production Wells No Violations On the day of inspection accompanied by ODNR Senior Health Physicist Paul Carder, I witnessed the well was online with the valves in the open position. Gauges present read 6995 pounds per square inches on the casing and 6170 pounds per square inches on the tubing with annular monitoring in place. Production equipment included a sand trap, a line heater and a 2- stage vertical separator. Also present included 4-400 barrel tanks within containment, 2 filter separators, 2 vertical scrubbers, 2 heater treaters and a gas combustor all shared with 34111246100000 Pittman 3H, 34111245910000 Holliday A 3H, 34111245960000 Holliday B 7H and 34111246090000 Circosta 1H. We met with Eclipse EHS Technician Ken Durst and completed walk around revealing containment was being removed. I instructed Mr. Durst to fill the mouse holes in accordance with OAC 1501:9-11-11 and to continue to pull fluids out of the sump until the levels are acceptable.
0860355257 2016-12-13 Completion Testing No Violations On the day of my inspection I witnessed flowback operations began this morning through Tetra Services flowback equipment. I met with Consultant Cristo Giannareas and completed walk around revealing the well was flowing back with 6500 pounds per square inches on the casing at 18.4 MMCF and 40 barrels per hour through a 24/64 inch choke.
1034079212 2018-05-22 Production Wells No Violations Status check inspection found the well drilling and completion process now complete and the well producing through 2.875 production tubing with 2 manual and 2 emergency shut down in line valves present in the production flowline. Proper well identification is posted. Annular pressure monitoring is in place on the 13.375 and 9.625 well head sections with 1,600 psi on the 9.625 and 0 psi on the 13.375 well head sections. Production related equipment on site consists of 5 gas processing in line heaters and 5 sand trap separators along with a single glycol heater. The production fluid facility consists of 4 steel construction tanks of 400 barrel capacity situated inside a polymer lined steel containment area.
1292455332 2016-12-21 Completion Testing No Violations On the day of my inspection I witnessed the well was flowing back with through Tetra Services flowback equipment to sales with 6300 pounds per square inches on the casing at 19.4 MMCF per day and 26 barrels per hour through a 25/64 inch choke. I met with Consultant Cristo Giannareas and completed walk around.
1335825227 2017-03-21 Preliminary Restoration No Violations Preliminary Restoration found the area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. The lease road is in good repair and stoned. The location entry point is posted with the owners contact information. The location is fenced with chain link fence. There is a total of 6 wells on the pad 5 of which are complete and all 6 have identification posted. The said well has a casing and tubing well head in place and is equipped with tubing head valves and B section cross valves. There is a Hydraulic operated electrically controlled emergency valve on the tubing head output flow line that has shut down points at the well head and production facility along with at the well pad entry point. Also on location downstream from the well head is a sand trap and line heater and then a mechanical separator. The mechanical separator sends the processed fluid to the production facility that consists of 4 steel construction 400 barrel capacity production tanks located inside a polymer lined steel containment area. There is also 2 dehydration units on location that service the 6 total line heaters on the pad.
1439988398 2015-08-19 Casing String Witnessed No Violations The said well was spudded on 08/17/2015 by Precision Rat Hole with a 30 x 120 conductor borehole being drilled and 120 of 24 .375 wall 94.62# pound per foot weld connection casing being ran. Universal Well Service is the cement contractor on location to cement the casing using 320 sacks of class A-3% calcium .25 pound per sack loss circulation material with the pipe capacity being displaced with 53 barrels of fresh water and 10 barrels of 6% gel prior to cementing with good cement witnessed to surface. Inspector Brian Sefcik accompanied me on this day.
1464262603 2016-05-26 Casing String Witnessed No Violations On site inspection to witness the 13.375 surface casing running and cement job. H&P Rig 531 is on location and has drilled a 17.5 surface borehole to a measured depth of 1,760 and 1,730 of 13.375 J-55 grade casing with a guide shoe and float collar ran and proper centralizer placement demonstrated. Schlumberger cement service is on location to cement the casing using 1,360 sacks of class A cement with circulation established prior to cementing and cement returns witnessed to surface and circulation maintained throughout the cement job.

1465841256 2016-06-13 Drill / Deepen / Reopen No Violations On site follow up inspection to a reported released of approximately 2 barrels of diesel fuel oil based drilling mud reported to me at 8:30 am by Bruce Carpenter with the company Eclipse resources. Upon inspection it was discovered the product was released when an employee closed the well bore discharge flow line valves while pressure was being exerted on the well bore causing a 150 psi rated plastic cap to fail on the outlet line (Elk Horn). The company is recovering the product by pressure washing the residue and using a vacuum recovery system. All product was contained on plastic containment and to the immediate working area.

2052243941 2016-06-07 Drill / Deepen / Reopen No Violations My inspection today finds the drilling contractor H&P is drilling an 8.5 borehole at a depth of 12,810. Last night the rig took a gas kick. The mud weight at the time of the kick was 12.4 pounds. The current mud weight is 13.4 and will be built to 14.2. Halliburton managed pressure drilling is also monitoring the weight. A five to ten foot flare is burning at the time of inspection.
