-110 -ICY -TSH W625 Internal connection casing shall be ran with a external coupling size of 5.242. The casing will consist of a 2 part string and the upper section from 5,500 to surface will be 23lb/ft P-110 ICY-TSH Blue with an external coupling size of 6.181. The total proposed well bore depth is 23,168. All drilling rig and support equipment is place on primary polymer containment.

0434380061 2015-07-15 Casing String Witnessed No Violations On this day I was on site for a 15:00 pm conductor cement job call out. Upon arrival I was notified that the drilling rig had broke down and the job was going to be pushed back until 09:00 am on 07/16/2015. The 32 borehole was drilled to 100 with no freshwater encountered and the drill line cable had broke causing the rig to not to be able to retreive the drill assembly from th rig out of the hole.

The Company has submitted a modified casing program in which has been approved to allow a 8.750 vertical borehole to be drilled and a 7.625 liner to be set from the bottom of the previous set 9.625 casing string to a measured depth of 9,000. The liner specifications are as follows- Total length of line ran 2,901.84 of P-110 grade 29.70# pound per foot with a Baker Hughes-hanger- packer isolation tool ran on top of the liner. A casing tally has been received.
1219341376 2015-10-26 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling process found Orion Drilling Polaris rig# 14 on the location with a 8.750 borehole drilled vertically to a depth of 9,000. Upon my arrival I found a Halliburton pressure pumping truck rigged up on the hole and attempting to squeeze the open hole section from below the 9.625 casing shoe set at 6,297, the company is pumping a LCM product through the drill string set at a depth near the wellbore TD at 9,000. No notification was received from the company (Stat Oil) or the company man on site of the up coming procedure. I did request both buy E-Mail and by phone to the company to post the Division of any upcoming activity.
1437782887 2015-07-24 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job found Orion Drilling Palaris Rig# 14 is now on location and has drilled a 24 surface hole to the depth of 436 and ran 411 of 20 94# Buttress thread casing with a float collar ran along with proper centralizer placement demonstrated. Halliburton Cement service is on location to cement the casing using 240 barrels of Class A (Bondcem) cement with circulation established prior to cementing and 40 barrels of 6% gel spacer ran ahead of the cement.

The company is batch drilling the # 2 @ #4 UH wells consecutively.
1446470478 2015-11-02 Drill / Deepen / Reopen Violations Noted Administrative Inspection to document the company failed to make direct notification to the field inspector 24 hours prior to the installation and cementing of the 7.625 casing liner as indicated on the issued drilling permit as a permit condition under inspector notification. On October 23rd of 2015 Stat Oil submitted a request to alter their casing program to the Divisions Permitting Section and received permission to proceed. On October 26th I visited the rig and requested to be kept in notification. On October 27th I spoke with George Manthos with Stat Oils Drilling Engineering section and informed Him the Division was to be informed as the project moved foreword. On 11/02/2015 - Spoke with the rig verbally and was informed the liner had been set and cemented without notification to the inspector prior to doing so. Corrective Action required -Moving Forward the company must follow their permit inspector notification section as indicated on their issued drilling permit and notify the inspector 24 hours prior to the Installation and cementing of all casing strings as indicated in OAC Chapter 1501:9-1-08 of the Well Construction rules paragraph (L) Notification. The owner shall notify the inspector at least twenty-four hours prior to setting any casing or liner string and before commencing any casing cementing operation pursuant to this rule to enable the inspector to participate in the pre-job safety and procedure meeting, independently test mix water , evaluate casing condition , and observe and document the execution of the cementing operation.

Halliburton Pressure Pumping Service is on location standing by to complete the last 2 stages on the well. Following the stimulation completion a snubbing unit owned by Deep Well Services will complete the well and prepare it for production.
