0099585492 2015-11-02 Drill / Deepen / Reopen Violations Noted Administrative Inspection to document the companies intention of running and setting a 7.625 casing liner without prior approval from the Divisions Permitting section. On this day I spoke with the drilling rig manager who indicated they were drilling a 8.750 diameter borehole in the vertical section below the 9.625 previous set casing string. In conversation with the Divisions Permitting Manager Steve Opritza it was indicated that Stat Oil had only submitted a plan for 3411124599-0000. Instructions are as follows- Cease operations of setting and cementing any casing strings on the said well immediately until a approved plan is submitted to, and approved by the Division of Oil and Gas Permitting Section. ODNR Monroe County Inspector Bill Stolarik 740-507 -6592.
1229681385 2017-06-20 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072 with vegetation and erosion control in place. The company has a 8 high chain link fence erected around the well pad and proper signage at the location entry point. Addressing the well status, the well drilling and stimulation process is complete with 50 stages stimulated. Halliburton Pressure Pumping Service is on location standing by to complete the last 2 stages on the 4-H well. Following the stimulation completion a snubbing unit owned by Deep Well Services will complete the well and prepare it for production.
1410541465 2015-11-05 Casing String Witnessed No Violations On site inspection to witness the cementing of the 7.625liner set to isolate the open hole section below the 9.625 casing string. Orion Drilling Polaris Rig #14 is on location and has drilled a 8.750borehole to a depth of 9,000measured depth. A 7.625 29.70 lb/ft hydrill 513 P-110 liner of 2,901.43 in length was then inserted into the well with a Baker Hughes ZXP-
1436891616 2015-07-14 Casing String Witnessed No Violations Status check inspection to witness the conductor pipe cement job found Well service group is the drilling contractor on location and has drilled a 32 conductor boreholeto a depth of 100 and ran 100 of 26 conductor borehole. Allied cement service is on location to cement the conductor using 262 sacks of Class A cement with circulation established prior to cementing and good cement returns witnessed to surface with the cement mix water tested for chlorides finding a 38 part per million level along with a PH of 7.
1437983065 2015-07-27 Casing String Witnessed No Violations On site inspection for a 12:30 am call out to witness the surface casing cement job. Upon inspection it was observed that Orion Drilling Polaris rig #14 is now on the hole and has drilled a 24 diameter surface borehole to the depth of 436 and ran 411 of 20 94# casing with a sting in float collar ran allowing the cement job to be implemented through the 5 drill string stung into the casing guide shoe-float collar. Halliburton Cement service was on location to cement the casing using 250 sacks of class A (Bondcem) cement with 40 barrels of 6% gel spacer ran ahead of the 102 barrel 15.8# 2.31yield cement mix with circulation established with the rig pumps prior to turning circulation control over to the cement crew. The cement mix water was verified free of chlorides and correct in PH levels. With good cement returns witnessed to surface.
1497128385 2016-06-07 Drill / Deepen / Reopen No Violations My inspection today finds a substantial area of plastic containment has been placed. Approximately 10% of the rig has been moved onto the location including the sub base.
1583629749 2016-07-19 Drill / Deepen / Reopen No Violations On site inspection accompanied by members of the Divisions Engineering and Radiology staff. Upon inspection it was found the Orion Drilling Polaris rig # 14 was on location and preparing to drill the 6.750 production borehole. A liner was previously set in the well bore below the 9.625 casing string reducing the production borehole drill size capability. The Engineering and Radiology staff was given a operation tour of the drilling rig back yard solidification area.

1918157930 2016-11-30 Completion Testing No Violations Status check inspection on the well completion process found a service rig on the well owned by Key Energy. The rig is fishing a 2.375 workover string lost in the well bore while attempting to clean out a open hole section outside the 5.1393 production string set to a depth of 21,360 with a 6.750 well bore total depth drilled of 25,006. Approximately 480-2.375 tubing joints totaling 14,000 with 1,600 outside the casing string in the open hole section.
