0178660740 | 2018-08-06 | Production Wells | No Violations | Status check inspection found 4 wells producing on the pad. The pad surface is of stone construction with a earthen perimeter berm, the perimeter valves are closed. The well head is equipped with a lower A and upper B section along with a production tree. The well is currently in production with the production valves open. The well head has riser pipes and valves with gauges present to monitor annular pressure on the A & B sections with 871 pounds per square inch pressure (psi) on the A section which monitors the pressure on the 13.375 diameter casing string (between the 13.375 and 9.625). The pressure on the B section which monitors the pressure on the 9.625 casing string (between the 9.625 and 5.5 diameter casing strings) is 710 pounds pressure. The 5.5 diameter production casing internal working pressure is 1,592 psi. The production flowline pressure is 1,084 psi. The well head assembly is equipped with 2 manual and one emergency shut down valve on the vertical section of the production tree followed by a single manual valve on the horizontal outlet section of the production tree. Also on location is 4 gas processing units along with a production facility tank farm consisting of 4 steel construction 400 barrel capacity tanks situated inside a polymer lined steel containment dike area. Proper well identification is posted at the well heads along with hazard and identification on the tank battery. At all critical hazard and muster points of the pad are fire extinguishers and emergency shut down master well control devices. A EQT midstream point is on the right front corner of the well pad. |
0279213101 | 2016-08-29 | Drill / Deepen / Reopen | No Violations | My inspection today finds Patterson Drilling 803 is drilling a 24 borehole at a depth of 174. |
0767952176 | 2017-03-09 | Completion Testing | No Violations | Status check inspection on the well completion process found Universal Well Service on location standing by on location while Allied-Horizontal Wireline Service perforates stage 36 of 39 total stages to be stimulated, the stage will be perforated with 48 perforations shot in 4 clusters between the depths of 11,760 and 11,610 with a completion plug to be set at 11,786. The well head completion stack consists of a Cameron Master valve and NOV completion stack, supported by 15,000 psi rated completion iron. All support equipment on location is located on secondary polymer containment with the well pad perimeter earthen berm serving as the primary containment with the well pad drain valves in the closed position on the day of my inspection. |
1474820735 | 2016-09-24 | Casing String Witnessed | No Violations | On site to witness running and cementing of 18.625 mine string Patterson rig 803 drilled to a depth of 740 and ran 708 of casing equipped with Tam Packer assembly and guide shoe and 9 centralizers and Allied cementing pumped 1076 sacks of Class A cement and they had 48 bbl. of good cement returns.
Packer was set at 519 and coal was encountered at 660-676 no mine encountered. |
1474986866 | 2016-09-27 | Plug / Plug Back | No Violations | On site inspection to witness the 17.5 1st intermediate well bore plug back process due to a lost hammer bit head at a measured depth of 2,130. Patterson Drilling rig 803 is on location and has run the 5.5 drill string to a measured depth of 1,000. Allied Cement service is on location and will set a open hole cement plug through the drill string consisting of 1,717 sacks of class A -1% calcium 16.2 ppg cement to be displaced to 1,000. ODNR Inspector Brian Sefcik was on site with me on this day. |

1980574833 | 2018-01-02 | Production Wells | No Violations | On site inspection as to a response to a report of a 25 gallon fluid release on the EQT midstream production pad that is situated on a site pad that adjoins the main well pad. The pad is situated on the South West side of the Switz 5 well pad and was depicted on the construction submittal plans in 2015. No product was visible on the earths surface and there was a roll off container on site with material place in it that had been recovered. Addressing the well, the drilling and completion process is now complete and the well is in production along with 3 additional wells on the pad. There is a total of (4) gas processing units on the well pad that the wells produce into. Aloes present is a production fluid storage facility consisting of (4) steel construction 400 barrel capacity production tanks situated inside a polymer lined steel containment dike atria. Proper identification is posted at multiple locations on site. |
