0392226934 2018-03-06 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction meets ORC 1509.072 with vegetation and erosion control in place. Addressing the well status- the well drilling -stimulation - completion and initial flow back process has been completed. The well is in production and is being produce off the 5.5 diameter production casing through the master valve and flow stack the flow stack consists of a primary master valve and 2 production valves along with a emergency shut down valve. The emergency shut down valve has several safety shut down buttons place on location and can also be shut down remotely through the SCADA computer system. At the rear of the well pad is (4) gas processing units and on the right (West) side of the well pad is the production facility consisting of (4) steel construction 400 barrel capacity tanks situated inside a polymer lined steel containment dike area. The entire well pad perimeter is fenced and gated with signage posted at the entrance point and on each well head.

The well is equipped with a production tree with a inline emergency shut down valve that can be controlled from multiple point on the well pad including the main entrance gate. The well is being produced into a gas processing unit and transferred off site through a main sales line. There is a total of (4) production tanks on the West side of well pad with a total capacity of 400 barrels each. The tanks are located inside a polymer lined steel containment area. There is a total of 4 active wells on the pad. Addressing the restoration, The area affected by the well pad construction has been restored with vegetation and erosion in established. The well pad perimeter drains are being monitored and tested for chlorides prior to releasing any surface water created on the pad from weather. No drilling pits were excavated during the well drilling and the entrance road is in good repair and accessible, the well pad perimeter is fenced with a primary gate in place and signage is posted at the well pad entrance.

On this day I contacted the drilling rig manager a one Dennis Mouser for a status update on the current recovery operations. Mr. .Mouser informed me they were currently milling on the top of the lost drill string and were going to attempt to recover the unit. I asked Mr. Mouser to keep me updated on the recovery status. Also noted was that no word had been received from the operations manager since my last inspection on 03/03/2016.

1256337402 2015-10-27 Casing String Witnessed No Violations The said well was spudded on 10/26/2015 by Well Service Groups top hole rig with a 100 x 36 conductor borehole being drilled and 100 of 30 diameter .375 wall conductor casing being set. Allied Cement service was on location to cement the casing using 270 sacks of class A-2% calcium .125 pound per sack loss circulation material with the cement displaced to 90 inside the casing with 75 barrels of fresh water.

There are (4) producing wells , (4) gas processing units, (4) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 220 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi. The production records are current.

1457626269 2016-03-10 Plug / Plug Back No Violations On this day I was on site to witness the setting of a deviation plug in the horizontal well bore section due to a lost drill string and drill assembly. The company initially reported on 03/01/2016 that they had twisted off their drill string at 14,024 and left 7,836 of drill string and bottom assembly in the hole, with a total depth drilled to 21,860. The company attempted to recover the lost drill tools in the hole unsuccessfully. A 600 deviation plug was set on this day 03/10/2016 through the 5.5 drill string with a 800 2.875 tail section below. The company will deviate around the lost tools and compete the well bore.

1467734539 2016-07-05 Completion Testing No Violations Status check inspection on the well completion process found the company is bringing in Deep Well Service tubbing snubbing unit to run workover tubing into the well bore and test the 5.5 production casing and drill out the mechanical bridge plug set in the vertical section of the well bore. The company will then run in the well bore and set a bridge plug above the casing shoe track and test the casing string. All workover support equipment will be on placed on polymer containment and the company has place the completion stack well assembly in place on the well. All well pad drain valves are in the closed position and the company is recovering any standing fluid on location.

The company initially set a spud data of 10/26/2015 but moved the date up to 10/23/2015 and on the morning of 10/26/2015 I discovered the wells had been spudded and drilled and were slated to be cemented.
