Gauges present read 7150 pounds per square inches on the casing and 6150 pounds per square inches on the tubing with annular monitoring in place. Production equipment included a sand trap, a line heater and a 2- stage vertical separator. Also present included 4-400 barrel tanks within containment, 2 filter separators, 2 vertical scrubbers, 2 heater treaters and a gas combustor all shared with 34111246100000 Pittman 3H, 34111245920000 Holliday A 1H, 34111245910000 Holliday A 3H and 34111245960000 Holliday B 7H. We met with Eclipse EHS Technician Ken Durst and completed walk around revealing containment was being removed. I instructed Mr. Durst to fill the mouse holes in accordance with OAC 1501:9-11-11 and to continue to pull fluids out of the sump until the levels are acceptable. |

FMC well head is on site and has reassembled the well head consist and pressure tested the production tree stack. Proper well identification is posted. Annular pressure monitoring is in place on the 13.375 and 9.625 well head sections with zero pressure on the 9.625 and 200 psi on the 13.375 well head sections. Production related equipment on site consists of 5 gas processing in line heaters and 5 sand trap separators along with a single glycol heater. The production fluid facility consists of 4 steel construction tanks of 400 barrel capacity situated inside a polymer lined steel containment area. |

Circulation was established during the running of the casing and maintained throughout the cement job with 20 barrels of 6% gel spacer ran ahead of the 1,334 sacks of class A cement. The casing was pressure tested to 500 psi over displacement pressure upon the landing of the cement plug with the floats holding and good cement returns to surface, the casing pressure test showed a initial pressure of 1,300 psi with a 75 psi loss in the 1st 5 minutes, it was observed that the cement head was leaking and the company was directed to test the casing when the BOP stack was installed. A casing pressure test was submitted with the BOP test the following day. |

The said well has a casing and tubing well head in place and is equipped with tubing head valves and B section cross valves. There is a Hydraulic operated electrically controlled emergency valve on the tubing head output flow line that has shut down points at the well head and production facility along with at the well pad entry point. Also on location downstream from the well head is a sand trap and line heater and then a mechanical separator. The mechanical separator sends the processed fluid to the production facility that consists of 4 steel construction 400 barrel capacity production tanks located inside a polymer lined steel containment area. There is also 2 dehydration units on location that service the 6 total line heaters on the pad. |
