The company men on location reported that the well bore had construction failure and in phone conversation with Jon Black with Antero Resources it was reported that the 5.5 diameter production casing had a joint split vertically at 2,200 measure depth from surface. It was reported that a bond log was ran and the top of cement was marked at 2,400 with a cement stringer at 1,100. Initially the toe sleeve in the 5.5 casing was opened and at that point the integrity failure was discovered. A proposal is being prepared by Mr. Black to the effect of plugging the well. My direct Supervisor Brent Bear along with the primary county Inspector Austin Guthridge was notified of the defective well construction. |

Emergency shut down devices are situated at the well pad entrance and at the tank battery loading site. Antero Production Superintendent Doug Stebbins was contacted on this day and met with me on site to discuss annular pressure. |
