During my inspection KLX Energy Services and Schlumberger were on location. KLX Energy Services ran a temperature log to locate the fluid level within the well. The log revealed a temperature change at 625 feet insinuating that is where the top of the fluid exist. Following the log Schlumberger rigged up and pumped a 300 foot plug of 91 barrels of class A cement at 16.4 pounds per gallon through the drill pipe. The drill pipe was removed and the plug was tagged at 720 feet. A second cement plug consisted 200 foot of 62.5 barrels of class A cement at 16.4 pounds per gallon. The top of the cement was tagged at 520 feet. The leaking casing shoe is now sealed. |

Universal Well Service cement company cemented the casing using 471 sacks of Type 1 cement with circulation established and 10 barrels of 6% gel ran ahead of he cement. Cement returns were witnessed at 60 barrels into a total of 91 barrels of fresh water displacement with cement visible inside the cellar and cement staying up to surface level. |

I left location at 7:30 pm with the company waiting on a wireline service to arrive. It was then reported by phone that the company went in with a weight bar and struck down on the DV tool allowing the stoke and establish circulation; the top section was then cemented. |

Multiple members of Eclipse Drilling Engineering staff contacted me by phone on the morning of September 10th 2016 and questioned my request to repair the failed casing shoe. I informed the company that they had a failed casing shoe on a mine string and the mine void encountered during the drilling of the well bore was not sealed and would communicate. Also noted was the large volume of gas encountered during the drilling of the mine string, the gas presence was significant enough to carry a flair until the top section was sealed. I explained to all parties that under 1501:9-1-08 of the well construction rules; under section E the casing shoe test could be required and remedial action be requested. |

Upon inspection of the well site it was observed that Halliburton Pressure pumping service was on location and standing by to stimulate stage 34 of 35 total stages to be stimulated. Nine Energy wireline service has perforated the 34th stage between the depths of 11,428 and 11,325 with 20 perforations shot in 4 clusters. The well is being stimulated through 15,000 psi pressure rated flow line iron with a FMC completion well head in place with both a manual and hydrologic controlled master valve in place on the well head. Annular pressure monitoring is in place on the well head annulis. All stimulation support equipment on location is placed on polymer containment with the well site earthen berm serving as the primary containment. |

The well pad perimeter is fenced with a 8 chain link fence and signage posted at the well pad entrance as well as at each well head and at the tank battery. Emergency shut down devices are located at the well pad entrance the tank battery and Gas processing units and also inside the remote transmission unit building, and finally at the well head. On location a total of (8) wells are present with all wells drilled- stimulated and completed with the production valves in the open position at the well head. Also present on the well heads is annular pressure monitoring gauges with zero pounds pressure showing between the 13.375 and 9.625, and zero psi between the 5.5 and 9.625. The company is monitoring and addressing any annular pressure. On the South side of the well pad is the production facility consisting of a total of (4) steel construction production tanks with a total of 400 barrels fluid capacity each. On the East side of the well pad is the production filtration and separation equipment consisting of a total of (8) gas processing units and (2 )dehydration units. Directly on the North East side of the well pad is the gas distribution point where the gas product is transferred over to Eureka Midstream. |
