0824360312 | 2017-08-31 | Preliminary Restoration | No Violations | Preliminary Restoration status check inspection found the well site meets ORC 1509.072 with the area affected by the well pad construction restored and vegetation established. The well pad perimeter is fenced with a 8 chain link fence and signage posted at the well pad entrance as well as at each well head and at the tank battery. Emergency shut down devices are located at the well pad entrance the tank battery and Gas processing units and also inside the remote transmission unit building, and finally at the well head. On location a total of (8) wells are present with all wells drilled- stimulated and completed with the production valves in the open position at the well head. Also present on the well heads is annular pressure monitoring gauges with 100 pounds pressure showing between the 13.375 and 9.625, and zero psi between the 5.5 and 9.625. The company is monitoring and addressing any annular pressure. On the South side of the well pad is the production facility consisting of a total of (4) steel construction production tanks with a total of 400 barrels fluid capacity each. On the East side of the well pad is the production filtration and separation equipment consisting of a total of (8) gas processing units and (2 )dehydration units. Directly on the North East side of the well pad is the gas distribution point where the gas product is transferred over to Eureka Midstream. |
0939336455 | 2016-10-28 | Drill / Deepen / Reopen | No Violations | During my inspection H&P Rig 531 was running the 5.5 inch diameter production casing to the proposed depth of 21,067 feet. Schlumberger was on location setting up to prepare for cementing. |

1469731800 | 2016-07-28 | Casing String Witnessed | No Violations | On site inspection to witness the conductor casing cement job. Precision Rat hole has drilled a 36 x 120 conductor borehole with no surface water encountered. 120 of 30 x .375 wall with weld connections conductor pipe was ran. Universal Well Service cement company cemented the casing using 405 sacks of Type 1 cement with circulation established and 10 barrels of 6% gel ran ahead of he cement. Cement returns were witnessed to surface with a total of 87 barrels of fresh water displacement with cement visible inside the cellar and cement staying up to surface level. |
1475496798 | 2016-10-03 | Casing String Witnessed | No Violations | On site inspection to witness the mine string -surface combination. Permission was granted by the Chief for the company to set the mine string deeper that 50. H&P drilling rig 531 is on location and has drilled a 24 diameter borehole to a measured depth of 903 with the mine shaft encountered at 789 to 800 and no fluid encountered in the mine. 18.625 casing was installed to a depth of 886 with a casing float shoe ran along with a float collar set in the string at 794 additional casing equipment consisted of cement basket at 732 and a inflatable packer set at 678 along with a stage tool set above the packer at 675 with proper centralizer placement being demonstrated with a total of 15 centralizers ran on the casing string. Schlumberger cement service cemented the casing using 430 sacks of class A cement on the bottom section below the mine void with the cement plug being deployed and displaced to the float collar with 233.3 barrels of fresh water, upon the landing of the plug the casing was pressure tested in increments up to 500 psi with the pressure being increased to 925 psi to open the inflatable packer, pressure was then decreased to 551psi to confirm the packer was set and then pressure was re applied to once again confirm the packer was inflated, pressure was the released and the DV tool open device dropped to open the DV tool sleeve and the 2nd plug inserted into the cement head, at this time the 2nd stage cement job was implemented with circulation being established to surface and 40 barrels of 6% gel spacer being pumped during the circulation cycle followed by 1,085 sacks of class A Cement displaced with 207 sacks of fresh water with cement returns being witnessed to surface. |
